Sarepta Therapeutics sells medicines for Duchenne muscular dystrophy, a fatal muscle-wasting disease of boys. It has two businesses. The first is three exon-skipping drugs (EXONDYS 51, VYONDYS 53 and AMONDYS 45). They have been sold for years and bring in a steady $945-967 million a year, about $800 million of it in the United States. The second is ELEVIDYS, a one-time gene therapy launched in 2023. It drove revenue to $1.9 billion in 2024 and $2.2 billion in 2025. In 2025 two non-ambulatory patients died of acute liver failure after treatment. Sarepta suspended shipments, and in November 2025 the label gained a boxed warning and lost the non-ambulatory population. Product revenue was $329 million in the second quarter of 2026, against $513 million a year earlier, which means ELEVIDYS now sells at a fraction of its 2025 pace. The cash record is the starting point. We measure what an owner could take out of the business each year: operating cash flow, excluding swings in working capital, less the cost of stock issued to employees and the capital spending needed to stand still. On that measure Sarepta lost money in nine of the last ten years. The losses ran from about $165 million to $620 million a year. The one exception was 2024, when it produced about $252 million ($2.34 a share). In 2025 the figure was minus $481 million. About $634 million of that was a one-time licence payment to Arrowhead Pharmaceuticals for an siRNA pipeline, and without it 2025 would have been about +$153 million. That year also included $175 million of one-off Roche income: a $63.5 million Japan approval milestone and a $112 million lapsed option. The first half of 2026 shows a $326 million profit, but almost all of it is a $325 million non-cash recognition of a Roche option Roche declined in February 2026. Operating cash flow for the half was roughly break-even. There is no multi-year level of positive cash earnings to anchor on. Our starting assumption is the most the restructured business has shown it can produce, about $150 million a year before interest. That figure comes from the 2025 underlying result after the July 2025 restructuring, which cut 36% of the workforce. We do not average in the years of heavy losses, because they describe a company that was still building its launch. Base case: the exon-skipping drugs hold roughly flat for a few years, ELEVIDYS sells only to ambulatory boys at today's lower rate, and cash flow before interest is about $150 million a year for two years, then fades to nothing by about year ten. The fade has named causes. In November 2025 the ESSENCE confirmatory trial for VYONDYS 53 and AMONDYS 45 missed its primary endpoint, which puts those accelerated approvals at risk. Dyne's DYNE-251 received Breakthrough designation in August 2025 and targets the same exon 51 patients as EXONDYS 51. Gene therapy is a one-time treatment, so each year's ELEVIDYS sales use up part of a finite pool of eligible boys. We credit nothing for the Arrowhead siRNA programs (DM1, FSHD and others), the LGMD gene therapies (SRP-9003 is on FDA clinical hold) or a return to non-ambulatory dosing. None of these has produced revenue, so none is in the bag. That stream is worth about $700 million. Against it, Sarepta owes $893 million of 4.875% convertible notes due 2030 (convertible at $60 a share) and $159 million of 1.25% notes due September 2027. Both carry rates below a long-bond return, which reduces their burden by about $42 million. It also owes $200 million of remaining guaranteed Arrowhead payments ($50 million a year to 2030). Against this it holds $788 million of cash and investments at 30 June 2026, of which we treat about $200 million as needed to run the business. Net obligations are therefore about $595 million. The base case leaves about $100 million for shareholders, roughly $0.96 a share. Bear case: the industry's own record for drugs approved on surrogate endpoints is the guide. After the ESSENCE miss, the FDA moves to withdraw VYONDYS 53 and AMONDYS 45, both part of the exon-skipping revenue. EXONDYS 51 loses share to DYNE-251, and ELEVIDYS demand keeps shrinking under the boxed warning. Cash flow before interest falls from about $60 million to losses of $40-80 million a year for several years before the company cuts back to break-even. The business is then worth less than nothing, about minus $200 million, while it still owes the $595 million. The company would owe roughly $790 million more than it is worth, about minus $7.50 a share. In practice that means a restructuring of the 2030 notes or heavy dilution, leaving shareholders little or nothing. Bull case: the restructured cost base holds, ELEVIDYS settles at a stable ambulatory volume, and the exon-skipping franchise keeps its approvals. Cash flow before interest is about $260 million a year at first and fades slowly over about sixteen years as competition arrives. That is worth about $1.8 billion, or about $1.23 billion after debt, roughly $11.66 a share. The previous figures are not comparable with these. Since then the second-quarter 2026 filing has shown how far ELEVIDYS sales fell, and the 2025 loss turned out to be mostly a one-time licence payment rather than operating decline. Neither change makes the outlook better on balance. Margin-of-safety price. We cannot name the earnings level with any confidence. Nine of the last ten years lost cash, the one good year depended on a product that has since been restricted, and the current profit is a non-cash accounting item. The worst plausible outcome is a regulatory one that no case fully captures: withdrawal of the accelerated approvals together with a further ELEVIDYS safety event. Sarepta's structure would not absorb it well. It depends on one disease and on one regulator, and it has $1.05 billion of debt coming due in 2027 and 2030. If we are wrong about the durability of the exon-skipping revenue, or about ELEVIDYS demand stabilizing, the shares are worth nothing. The business pays no dividend while waiting. Even the median case leaves only about a dollar a share, so there is no price at which we can be confident of a 20% annual return. No margin-of-safety price is set.

Chart analysis
A chartist would see a long-term downtrend: shares collapsed from the 2024 highs above $150 after the 2025 patient deaths and shipment suspension, gapping down repeatedly on heavy volume. The stock is well below its falling 50- and 200-day moving averages, with prior support levels now acting as overhead resistance. Any base-building at the lows would need a sustained move above the 200-day average before the trend could be called reversed; until then rallies are read as counter-trend moves in a news-driven, high-volatility name.
