Business condition attached to the margin of safety Sephience's launch continues to gain reimbursement and prescriber uptake in its major markets for two or three more quarters without a setback, and there is more clarity on how much of Translarna's European business survives on a country-by-country basis.
PTC Therapeutics is a rare-disease drug company in the middle of replacing its entire commercial engine. For most of the last decade the company lost significant cash every year funding research while building out two older medicines - Translarna, for a rare form of Duchenne muscular dystrophy sold mainly in Europe, and Emflaza, for Duchenne in the United States - alongside a valuable royalty on a third party's spinal muscular atrophy drug (Evrysdi) that PTC helped discover. Over 2024 and 2025, nearly every piece of that older engine changed. The European Commission declined to renew Translarna's marketing authorization in March 2025, putting most of its European sales at serious risk. Emflaza's US market protection began lapsing in February 2024 and the remainder expires in June 2026, opening it to generic competition. And PTC sold the last of its Evrysdi royalty at the end of 2025, banking roughly $2.1 billion in total proceeds but giving up any further income from that drug. In their place, PTC is now launching two new medicines: Sephience, approved in mid-2025 for phenylketonuria, and Kebilidi (also sold as Upstaza), a gene therapy for a very rare genetic disorder called AADC deficiency. PTC also out-licensed its Huntington's disease program to Novartis in a deal that closed in January 2025, collecting a $1.0 billion upfront payment. That $1.0 billion payment is the reason PTC's reported 2025 results look spectacular. We did not treat that as PTC's ongoing earning power - it was a one-time payment for handing off a program, not a recurring result, and once it is set aside, PTC's underlying 2025 cash burn was almost identical to 2024's. Our estimate is built on that underlying, ongoing level of cash generation, which we treat as having been in the range of a $3.50-$4.00-per-share loss most recently, and then on a view of how that changes over the next decade as Translarna and Emflaza fade and Sephience and Kebilidi take their place. In our base case, PTC's cash losses narrow steadily over the next few years as Sephience reaches a full year of global sales and the cost of running the Huntington's program shifts to Novartis, turning solidly cash-generative within three to four years - consistent with early signs already visible in PTC's most recent quarterly results, which turned solidly profitable in 2026 without the help of any one-time payment. On that path, we estimate the shares are worth around $38. Our bear case assumes Sephience's launch disappoints, Translarna's European sales collapse faster than expected, Emflaza's US decline plays out like a typical branded-to-generic transition, Kebilidi's uptake stays minimal given how rare its target disease is, and the Huntington's partnership produces no further payments beyond the money already banked. In that scenario, on a pure earnings basis, we estimate the ongoing business would be worth less than zero - roughly $6 per share underwater. To be clear, this does not mean PTC is at risk of running out of money: it finished 2025 with about $1.9 billion in cash and investments against modest debt, more than enough to fund years of the kind of losses we're describing. What it does mean is that if the new medicines fail to deliver, the recurring business would be spending shareholders' money faster than it makes it. Our bull case assumes Sephience becomes a genuine rare-disease blockbuster, Kebilidi's adoption exceeds expectations because of its very high per-patient price, Translarna retains meaningful sales in several European countries through national exceptions, and the Huntington's program achieves positive clinical results, triggering some of the up-to-$1.9-billion in additional payments from Novartis. In that case we estimate the shares could be worth roughly $150. A large share of every one of these estimates - on the order of two-thirds to three-quarters - comes from what we expect the business to be earning many years from now, rather than from what it earns today. That is typical of a company whose main new products only just launched, but it means these estimates are more sensitive than most to whether Sephience and Kebilidi prove to be durable franchises rather than one more cycle in a pattern PTC has now lived through twice, where a product built market share only to lose its protected position within a few years. Given how wide that range is and how much rests on unproven execution, the margin-of-safety price is set well below our base-case estimate - around $19.
