Insulet makes the Omnipod, a tubeless insulin patch pump for people with insulin-dependent diabetes. The current generation, Omnipod 5, is an automated insulin delivery system that adjusts insulin dosing continuously using a Dexcom or Abbott glucose sensor; it is now available in 19 countries and, since August 2024, is FDA-cleared in the U.S. for type 2 as well as type 1 diabetes. Because the Pod is a disposable worn every few days, once a patient converts to Omnipod the revenue is recurring and highly predictable, which is the central feature of this business. Over the past decade the company built out manufacturing capacity in Massachusetts, Costa Rica and Malaysia well ahead of the demand it now serves; capital spending as a share of revenue peaked around 2018-2019 and has since fallen sharply as volume caught up with capacity. That earlier capacity-building period produced little to no owner earnings even though the company was investing soundly for its future; the business has really only turned into a strong cash generator since Omnipod 5 scaled, with owner earnings per share rising from roughly break-even in 2020 to about $3.54 in 2023, $4.05 in 2024 and $5.80 in 2025. We anchored the valuation on the median of the last four fiscal years (2022 through 2025), a period bookended by Omnipod 5's original FDA clearance, which gives a normalized starting point of about $3.80 per share rather than simply extrapolating the most recent, unusually strong year. That most recent year benefited from two items that are unlikely to repeat at the same scale: a large non-cash charge from retiring $1.05 billion of convertible notes (replaced with a fixed-rate bond and a term loan), and a swing in deferred taxes versus the prior year. Stripping those distortions out, we assumed first-year growth in the base case well below the raw trailing growth rate, building back up from the underlying volume and geographic drivers instead. Three cases: Bear case: growth slows to low single digits and then modestly reverses. This reflects named, concrete risks: a small number of distribution partners account for the large majority of receivables (the three largest represented about two-thirds of trade receivables at the end of 2025), competitors including Medtronic's diabetes unit (being spun off as an independent company), Tandem Diabetes Care, and newer entrant Beta Bionics, and the possibility that GLP-1 medications slow the rate at which type 2 patients progress to needing insulin pumps. The company also carries about $950 million of net debt, including $450 million of 6.5% notes due 2033, so a period of weak growth would coincide with a heavier relative interest burden. Base case: continued, moderating growth as the U.S. base keeps expanding, the newer type 2 indication (only just cleared) is adopted gradually, and international markets continue converting from the older Omnipod DASH to Omnipod 5, with growth easing from roughly 20% to high single digits by year ten as the more obvious conversion opportunities are worked through, then holding roughly flat in real terms after that on the strength of the recurring-revenue base and the company's patent position. Bull case: the type 2 indication and international rollout scale faster than the base case assumes, and the operating leverage already visible in the numbers (selling and administrative costs fell from 44.3% to 43.0% of revenue in 2025, gross margin rose from 69.8% to 71.6%) continues, supporting mid-single-digit real growth even after the ten-year horizon. What sustains value beyond the explicit ten-year forecast is the recurring nature of the Pod itself, a patent estate of more than 1,000 issued U.S. patents (with expirations staggered from 2026 through 2047, so there is no single cliff) plus over 700 pending applications, and a nearly two-decade record of continuous product iteration from the original Omnipod through Eros, DASH, Omnipod 5 and now Omnipod 6 in development. We treated that as support for holding today's earning power rather than as license to keep assuming double-digit growth forever. Before finalizing this, we tested the assumption that margins keep expanding: the company's own filing notes that certain medical devices currently carry a tariff exemption on Malaysia- and other internationally-sourced products, and that if that exemption were removed, tariffs "would have a material impact on our results of operations in future years." That is a real, disclosed risk to the margin-expansion assumption embedded in the base and bull cases, and it is one reason the bear case assumes margins stop improving rather than assuming they keep rising indefinitely. Putting these cases through a ten-year model gives values of roughly $57 per share in the bear case, $162 in the base case, and $304 in the bull case. Given the size of that range, and the real (if not catastrophic) risks named above, the margin-of-safety price is set meaningfully below the base-case estimate — around $65 per share — a level close to the bear-case value, with the base and bull cases implying substantial upside from there.
