Business condition attached to the margin of safety Medicare's new CGM competitive-bidding contracts are published (structure and initial awarded pricing, expected around the 2027 contracting process ahead of the January 2028 payment change).
Dexcom makes continuous glucose monitoring (CGM) sensors and receivers used mainly by people with diabetes, plus a newer over-the-counter biosensor, Stelo, launched in August 2024 for adults with prediabetes and Type 2 diabetes who don't use insulin. Revenue has grown from roughly $573 million in 2016 to $4.66 billion in 2025, though the growth rate has been uneven: about 25% in 2023 as the newest sensor generation (G7) ramped, slowing to about 11% in 2024 amid heavier competition and pricing pressure, then picking back up to about 16% in 2025 as Stelo scaled and international sales grew. We build our estimate of value from the cash the business actually generates for owners: operating cash flow, minus the swings tied to working capital, minus stock-based compensation (a real cost to shareholders even though it doesn't cost cash directly), minus the capital spending needed just to maintain the current manufacturing base (separating that from the extra spending tied to growing capacity). Over the last three full years, that figure has averaged a bit over $1 per share, after recognizing that one of those years was held down by heavy legal costs from patent litigation with Abbott (settled in December 2024) and another was lifted by a one-time tax benefit and gains on investments. We use the middle of that range as our starting point rather than either extreme. From there we build three distinct pictures of how the business could unfold over the next decade and beyond, using the same fixed discount rate we use for every company, because we think risk belongs in how each story is built, not in the discount rate. The base case assumes Dexcom keeps growing owner earnings per share at a mid-teens rate over the next two years as Stelo continues to scale and the newest sensor (the G7 15 Day, cleared in late 2025) extends Dexcom's wear-time advantage internationally. In 2028, Medicare is set to bring CGMs into its competitive-bidding program for the first time, with new pricing taking effect January 1, 2028; we model a real step-down in growth that year to reflect that. Growth then settles into the high single digits before gradually fading toward a flat, no-growth level roughly fifteen years out, once we can no longer point to a specific reason for further gains. On this path, we estimate the shares are worth around $37. The bear case assumes the pricing pressure Dexcom has already disclosed, from higher rebate costs and channel mix, continues, and that the 2028 Medicare pricing change lands on the harsher end of what has been proposed, meaningfully cutting reimbursement in Dexcom's largest single payor category. It also assumes Stelo's growth is slowed by direct competition from Abbott, which markets its own biosensor into the same non-insulin market. In this case, growth is minimal for most of the next decade, and we estimate the shares are worth around $18. The bull case assumes Dexcom's own visible growth levers play out well: Stelo penetration accelerates given the still largely untapped population of people with prediabetes and Type 2 diabetes who don't currently use insulin, the G7 15 Day supports continued market-share gains internationally, and the company keeps returning capital to shareholders through buybacks at a pace similar to the roughly $500-750 million a year it has repurchased in each of the last three years. On this path, we estimate the shares are worth around $49. One accounting judgment worth noting: Dexcom's cash flow includes large non-cash add-backs for depreciation, which is mostly manufacturing equipment rather than acquired intangibles, and in 2025 specifically, a sizable tax benefit and gains on investments that are not part of normal operations. We left those in the reported cash flow, which is standard, but we did not let the unusually strong 2025 result set the pace for the future - the growth we assume going forward starts well below the roughly 125% jump owner earnings showed in the most recent year, since most of that jump came from items unlikely to repeat. Given this range, the margin-of-safety price sits meaningfully below our base-case estimate to leave room for the downside risk described above, at roughly $24 or lower. How Medicare's new CGM bidding program actually prices out, expected around 2027, is the single decision that affects a large share of the downside case.
