Globus Medical makes spinal and orthopedic trauma implants, surgical instruments and the ExcelsiusGPS robotic navigation system, and since April 2025 the Nevro spinal-cord-stimulation devices for chronic pain. It merged with NuVasive on September 1, 2023, roughly doubling in size, and bought Nevro on April 3, 2025 for $5.85 per Nevro share in cash (the accounting produced a one-time $117.7 million bargain purchase gain, which we exclude). Revenue was $1.57 billion in 2023, $2.52 billion in 2024, $2.94 billion in 2025 and $1.55 billion in the first half of 2026 (up 15% on the first half of 2025). The company pays no dividend but has bought back stock: $300 million in 2025 and $136 million in the first half of 2026. How we measured earnings. We start from operating cash flow, remove working-capital swings, and subtract stock compensation and the capital spending needed to keep the business running. Per diluted share this gives $1.22 (2016), $1.32, $1.53, $1.51, $1.45, $2.10, $1.77 (2022), $2.00 (2023), $3.25 (2024) and $4.42 (2025). For 2023 to 2025 we treated all capital spending as maintenance because the revenue growth came from acquisitions, the conservative reading. Cross-checking 2025 from the other direction (net income of $537.9 million, plus $276.8 million of depreciation and amortization, plus other non-cash items netting to about $5 million, less stock pay and $164.7 million of capital spending) gives the same $606 million. Amortization of acquired NuVasive and Nevro technology runs about $118 million a year (about $0.86 per share) and is added back as a sunk acquisition cost; because those acquired product lines wear out, the growth we allow is limited to the company's own product-development record rather than assumed to continue indefinitely, and every case fades to nothing in its far years. Earnings level. Only 2024 and 2025 reflect the combined company, and their median is $3.84. The first half of 2026 now corroborates a higher level: operating cash flow of $412.1 million less $72.8 million of capital spending and $26.0 million of stock pay is $313 million, about $4.55 per share annualized before any working-capital adjustment. With 2025 at $4.42 and two further quarters running at that pace, we use $4.20 per share (about $577 million) as the base level, between the two-year median and the current run rate. This is the main change since our previous analysis: two more quarters of results confirmed that 2025 was not a one-off. Growth check. Owner earnings per share rose 36% in 2025, but that reflects the Nevro addition and merger savings, not a repeatable rate. The base case assumes 3% in year one, far below that. The pre-merger company grew owner earnings per share from $1.22 in 2016 to $1.77 in 2022, about 6% a year, funded entirely from its own cash and held through the 2020 shutdown ($1.51 in 2019, $1.45 in 2020). Base case, $70.98 per share. Earnings of $4.20 per share grow 3% a year for ten years, hold flat for ten more, then fade to zero over the following ten. The 3% is about half the company's own 2016-2022 record of steady new-product introductions, the only growth we can show is repeatable. We leave out robot placements beyond today's, share gains from the NuVasive sales force, Nevro recovery and further margin improvement, because none is secured. The business is worth $65.63 per share; it owes nothing in borrowings and holds net cash of $5.34 per share, giving $70.98. Bear case, $39.94 per share. This case includes a full downturn of the kind the industry has actually had. In the 2020 shutdown elective spine surgery stopped and Globus's second-quarter revenue fell 23.5% from a year earlier ($148.9 million against $194.5 million). We start the bear case below the base level at $3.40 (closer to the 2024 figure), apply a deeper, longer hit with earnings at $2.40 in year two, recover to only $3.00 and then $3.25 (well under the base level, reflecting hospital price pressure and loss of acquired surgeon relationships), hold there through year ten, then erode 3% a year through year twenty, after which we credit nothing. Two disclosed risks sit behind it: an FDA warning letter dated July 16, 2024 concerning complaint handling for ExcelsiusGPS, still open at February 24, 2026, and the integration of two large acquisitions inside two years. The business is worth $34.60 per share; with net cash of $5.34 the shares come to $39.94. Bull case, $100.93 per share. It starts from the current run rate of $4.50 and grows 6% a year for ten years, matching the company's own 2016-2022 record, then holds flat for fifteen years and fades over ten. The business is worth $95.58 per share; with net cash of $5.34 the shares come to $100.93. Debt and cash. The $450 million of 0.375% convertible notes inherited from NuVasive were repaid in 2025, leaving no borrowings of note and no meaningful interest bill, so no interest was added back to earnings. At June 30, 2026 the company held $507.7 million of cash, $87.4 million of short-term and $245.4 million of longer-dated marketable debt securities, $840.5 million in all. Against that it owes $105.4 million of contingent acquisition payments and a finance lease under $1 million. Net cash is $734 million, $5.34 per share on 137.4 million diluted shares, applied equally in every case. Our previous analysis left out the longer-dated securities, which is why the cash figure is higher here. What would make this wrong. If the FDA matter escalates to import or shipment restrictions on ExcelsiusGPS, if NuVasive or Nevro surgeons drift to competitors, or if the 2025 and 2026 cash flow proves flattered by timing, earnings would settle nearer the bear level. Capital spending more than doubled from $78.3 million in 2023 to $164.7 million in 2025; we deduct all of it, so if it falls back our figures are conservative. Margin of safety price, $41. We are reasonably sure of the earnings level: the combined company has two full years plus two corroborating quarters, its cash flow cross-checks against reported earnings, and the pre-merger business held its earnings through the 2020 shutdown. The worst plausible outcome no case models is a serious FDA action that halts robot shipments while integration stumbles, causing surgeons to move to Medtronic or Stryker; the structure would survive it because there is no debt to refinance, no customer concentration and $840 million of cash and securities, but earnings could sit below the bear level for several years. If we are wrong on both the earnings level (it proves closer to $3.40 than $4.20) and on growth (none at all), the shares would be worth roughly the low $50s; in the bear case, about $40. The price assumes a holding period of about three years for value to be recognized; the business pays no dividend, so any return has to come from retained cash and buybacks building value per share. The margin-of-safety price of $41 is roughly matched by the bear-case value, and from that price the base case implies about a 20% annual return over three years. Above that price the base case implies less than the 20% target.

Sales geography
Primarily the United States (Northeast, Midwest, South and West); international sales in Europe, Japan, Australia, Latin America and other countries.
Historical growth strategy
Organic growth from a steady stream of internally developed spine and orthopedic implants sold through a direct, surgeon-focused sales force, extended by the ExcelsiusGPS robotic navigation platform that pulls implant volume; since 2023 supplemented by large acquisitions (NuVasive in 2023, Nevro in 2025) paid for from its own cash and stock, while carrying essentially no debt.
Chart analysis
A chartist would describe a long, choppy range rather than a trend: the shares broke down from the high $70s-$80s in 2022 to the mid-$40s around the NuVasive merger in 2023, staged a strong recovery to new highs near $90 in early 2025, gave much of it back in a sharp mid-2025 drop toward the $50s, then rebuilt into the $80s during 2026. The repeated failures near $90 mark that level as heavy resistance, with the $50-$55 area as established support; a decisive close above the prior highs would be read as a breakout from a multi-year base, while a loss of the mid-$60s would suggest the range is reasserting itself.
