Business condition attached to the margin of safety Reported operating cash flow turns positive for at least three consecutive quarters (after stock compensation), showing the business funds itself without new equity or debt.
Biodesix sells lung-cancer diagnostic tests (Nodify Lung nodule risk tests, IQLung and Lung Cancer Detection/Genomics testing) and biopharma laboratory services, almost entirely in the United States. Revenue has grown from $45.6M in 2020 to $88.5M in 2025, but the business has not yet generated positive cash. Our measure of owner earnings (operating cash flow, less working-capital swings, less stock compensation, less maintenance capital spending) was about -$38M in 2021, -$52M in 2022, -$46M in 2023, -$55M in 2024 and -$29M in 2025, or roughly -$3.81 per share in 2025 after the 1-for-20 reverse split in September 2025. The 2024 figure includes roughly $23M of payments of contingent consideration on past acquisitions. Losses are narrowing: the fourth quarter of 2025 net loss was $4.0M versus $35.3M for the full year. Base case: worth about $0 per share. The trailing owner earnings are negative, and nothing in the filings secures a move to positive earnings; revenue growth and margin gains are not contracted or a long-proven process, so we do not credit them. Bear case: about -$3 per share. The company has a Perceptive term loan (about $62M of principal and interest due mostly in 2027-2028) against $19.0M of year-end cash plus ATM proceeds; if losses persist, refinancing or repayment leaves the company owing more than the business is worth, by a few dollars per share, and further share issuance would dilute holders. Bull case: about $9 per share. Here the recent trend continues: cash burn falls to breakeven by about 2027 and owner earnings reach roughly $15M by 2030, then persist for a decade with dilution from further capital raising. That case rests on the demonstrated improvement of the last two years but requires results that have not yet been reported. What could make this wrong: the business is closer to breakeven than the trailing figures suggest (Q4 2025 loss was much smaller than the annual average), and the company regained Nasdaq minimum-bid compliance in September 2025 after the reverse split. Continued quarterly improvement would raise our estimate; renewed cash burn or reimbursement cuts would lower it. The margin-of-safety price is very low and applies only after the company shows sustained positive operating cash flow.
