Alto Neuroscience is a clinical-stage drug developer with no revenue and no products on the market. It uses brain-based biomarkers to try to match psychiatric patients to treatments; its lead candidates (ALTO-100, ALTO-203, ALTO-207 and ALTO-300) are in mid-stage trials. The business consumes cash and earns nothing today: owner earnings were about -$36M in 2023, -$63M in 2024 and -$63M in 2025 (-$9.76, -$2.56 and -$2.19 per share; the 2023 share count is distorted because the company listed in 2024). Operating cash burn has since stepped up, to about -$47M in the first half of 2026 (annualising near $95M). Balance sheet: cash was $243.7M at 30 June 2026 (after a raise earlier in 2026 lifted it from $176.5M at year-end), against roughly 43M shares, or about $5.70 per share. We found no borrowings in the data available. At the current burn that cash covers roughly two and a half years. We could not open the full filing text for this run, so working-capital, share count and debt figures rest on the structured financial data. How we valued it: shareholders own the cash less what the company will spend before it learns whether its drugs work. There is no contract, approval, revenue or repeated record of successful launches that we can point to, so we assign no value to the drugs themselves. Published early-stage data for the lead programme is encouraging, but it has not yet produced a Phase 3 result, and that is not evidence we can bank. Our figures are therefore what the cash is worth after the trial programme is paid for. Bear (about $0.50/share): trials read out negatively in 2027, but spending has already run above the current pace (about $110M a year as trials enlarge, a pattern common to psychiatric programmes, where failed Phase 2 and 3 trials are the industry norm). Wind-down costs and lease commitments consume most of what remains, leaving little for owners; the business is worth roughly the leftover cash and owes nothing, so the equity retains a small residual. A further equity raise at depressed prices would dilute holders further. Base (about $1.50/share): the company spends about $95M a year through 2027 and 2028 readouts, with the typical psychiatric-drug outcome of failure or inconclusive data on at least the main programmes. Roughly $180M of today's $244M is consumed in present-value terms, and about $10M more goes on winding the programme down, leaving about $65M, or about $1.50 per share. The 2025-2026 raise supports the view that the company can fund this path without borrowing. Bull (about $3.20/share): early failures lead management to stop spending after about one year of trials (roughly $95M), leaving about $150M of cash less wind-down costs. We deliberately leave out the value of a successful drug, because nothing has yet been proven in late-stage trials; a positive Phase 3 result would be worth far more than any figure here, and is the reason the shares can trade well above these cases. What changed since our previous estimate (August 2026 range of $3 / $10 / $30): that estimate gave credit for pipeline success that is not yet demonstrated, so the figures are not comparable, and the lower values here reflect only that unproven drug value is now excluded. Business facts moved in both directions: cash rose to $243.7M but spending also accelerated and the share count rose from about 32M to about 43M. What would make this wrong: a positive Phase 2b/3 readout on ALTO-100 or ALTO-300 would transform value; sharply slower spending would extend runway and lift every case. Margin-of-safety price: earnings are negative and the outcome is binary. The most likely result for a psychiatric drug programme is that trials disappoint and owners recover only part of today's cash after years of spending, so a margin-of-safety price near $1.00 sits below the base outcome ($1.50), while the bear outcome ($0.50) implies a loss from that price. The price assumes a holding period of about two years for readouts; the business pays no dividend in the meantime.

Sales geography
No product sales. Pre-revenue; clinical trials conducted in the United States (West, Northeast, South and Midwest sites).
Historical growth strategy
Funds an internal, biomarker-guided drug discovery platform (precision psychiatry) through venture rounds, an IPO in early 2024 and follow-on equity raises; advances each candidate through Phase 2 on its own, with no acquisitions, no revenue and no debt financing evident.
Chart analysis
A chartist would see a recent-IPO biotech whose shares fell hard from the 2024 listing, then rebased in a wide, volatile range driven by clinical-trial news. Trading is headline-driven rather than trend-driven, with sharp gaps on data and financing events and support/resistance levels that reset after each equity raise. Without a sustained higher-low sequence there is no confirmed uptrend, and any breakout would need volume confirmation on a trial readout.
