Ocular Therapeutix is an eye-care drug developer with one marketed product and one late-stage drug candidate. The marketed product, DEXTENZA, is a small dissolving insert placed in the tear duct after cataract surgery to control pain and inflammation. It is sold only in the United States. The company's value mostly depends on AXPAXLI, an implant injected into the eye. It slowly releases the drug axitinib to treat wet age-related macular degeneration (wet AMD). The business has never made money. Owner earnings (operating cash flow excluding working-capital swings, minus stock-based pay and capital spending) have been negative every year for a decade. They were about -$1.00 per share in each of 2020 to 2024 and -$1.42 per share in 2025, when the company spent -$267 million. Because there is no positive earnings record to anchor on, we valued the company on what it owns today and what it has already committed to spend. At June 30, 2026 it held $598.6 million in cash and owed $82.5 million on a secured Barings term loan due in 2029. Management says the cash lasts into 2028 on current plans, and that figure excludes the cost of launching AXPAXLI. DEXTENZA revenue fell from $63.5 million in 2024 to $51.8 million in 2025, even though units sold rose about 5,000 to roughly 180,000. The cause was discounting: deductions from the gross price rose from 38.5% to 51.6%. The company raised its discounts again in January 2026 and expects deductions to stay at this level or rise. Second-quarter 2026 revenue was $13.5 million, flat with a year earlier. In 2025 the company spent $53.9 million on selling and marketing, which already exceeds DEXTENZA's gross profit. On its own, DEXTENZA contributes little. It also depends on Medicare paying for it separately from the surgery. The filings say that payment is reviewed every year, and the current statutory provision is time-limited. If it ends, the company says DEXTENZA revenue would decline significantly. AXPAXLI has strong results, but it is not yet an earning asset. In the SOL-1 Phase 3 trial, 74.1% of AXPAXLI patients maintained vision at week 36, compared with 56.6% on aflibercept (Eylea). At week 52 the figures were 65.9% and 44.8%. In May 2026 the FDA confirmed that this single trial can support an application, and the company plans to file in the fourth quarter of 2026. The second Phase 3 trial, SOL-R, fully enrolled 640 patients and reports in the first quarter of 2028. A trial in diabetic retinopathy, HELIOS-3, is enrolling. However, AXPAXLI is not approved, has no sales, and would have to win share in a market held by Eylea and Vabysmo. None of that has happened yet, so no case below counts any AXPAXLI revenue. We count the spending needed to reach the SOL-R readout, because that spending is under way. We do not count the payoff, because it depends on an approval and a launch that have not occurred. This is the main reason our values are far below what the market may pay for this pipeline. All cases discount at 6%. Base case, about $0.36 per share: the company spends about $450 million in cash between mid-2026 and the SOL-R readout in early 2028. It then holds roughly $150 million, or about $65 million after repaying the Barings loan. DEXTENZA adds a small ongoing contribution of about $3 million a year. Bear case, about -$0.04 per share: spending keeps rising at the first-half 2026 pace, when selling costs rose 26% and administrative costs rose 55% year over year. Cash falls to roughly the size of the $82.5 million loan by the readout. Medicare's separate payment for DEXTENZA ends after 2027, and the product stops contributing. Here the debt slightly exceeds everything the business owns, by roughly $9 million, before any new financing. Bull case, about $0.79 per share: spending stays closer to the 2025 level, leaving about $200 million in cash at the readout. Separate payment continues, and DEXTENZA's unit growth turns into about $10 million a year of contribution. The per-share figures use about 225 million shares. The company has issued large amounts of stock repeatedly: diluted shares rose from 25 million in 2016 to about 225 million in 2026, and launching AXPAXLI would likely require more. The low value does not mean AXPAXLI is worthless. It means its worth cannot yet be shown from anything the business has earned. The value would change materially if the FDA approves AXPAXLI and the drug then shows real sales against the established wet AMD treatments. Not reconciled: the filings do not break out DEXTENZA's selling costs separately from launch preparation for AXPAXLI. Our estimate of DEXTENZA's standalone contribution is therefore a judgement.
