Business condition attached to the margin of safety No specific business condition is required beyond the price itself; conviction would improve if the core testing franchise gets through one more full fiscal year without an adverse Medicare coverage or reimbursement-rate action, given the precedent of a related test losing its Medicare reimbursement in 2025.
Interpace Biosciences runs a single molecular diagnostics franchise today: two tests that help physicians decide whether an indeterminate thyroid nodule needs surgery. The company sells almost entirely through a major national lab partner and is reimbursed mainly by Medicare. This is a much smaller, more focused company than it was a few years ago: it sold an unrelated pharmaceutical-services business in 2023, and in April 2025 the Centers for Medicare & Medicaid Services stopped reimbursing its pancreatic-cyst test, which the company then discontinued. The upside of this narrowing is real — the company has no debt, a healthy cash position, and has generated positive and growing operating cash flow for three straight years after years of losses. The downside is concentration: nearly all revenue now depends on one test family and one payer's coverage decision, and the discontinued pancreatic test shows that risk is not hypothetical. Our base case assumes the remaining drag from that product's wind-down finishes working through the numbers this year and the core thyroid-testing business then grows slowly and steadily for years to come, supporting a value of roughly $2.64 per share. Our bull case assumes the core business grows a bit faster through expanded outreach and stable reimbursement, worth roughly $3.70 per share. Our bear case assumes a further reimbursement setback or competitive loss — echoing what happened to the discontinued test — that shrinks the business sharply before it stabilizes at a smaller size, worth roughly $0.65 per share; even in this case we do not see the company as worth less than nothing, since it carries no debt. The margin-of-safety price is around $1.32 per share, a substantial discount to our base-case estimate that reflects the risk that this is now essentially a one-product company. One accounting note: a recent year's reported net income was much higher than its cash flow because it included a one-time, non-cash tax benefit from releasing a valuation allowance on old tax losses; we did not treat that as ongoing earning power. We also note that all of the company's preferred stock recently converted into common stock, leaving a cleaner capital structure than the company has had in years.
