Tactile Medical makes at-home compression therapy devices for lymphedema and chronic venous insufficiency (its Flexitouch and ACTitouch systems), and, since acquiring the AffloVest product line in September 2021, airway-clearance vests for chronic respiratory conditions like bronchiectasis and COPD. Devices are billed mainly to private insurers, Medicare, the Veterans Administration and durable-medical-equipment distributors, with garments and related supplies replaced periodically, giving the business a recurring, consumable-driven revenue base rather than a one-time sale. The AffloVest acquisition meaningfully changed the company, so we measured normal earning power from the first full years with both product lines together (2023-2025) rather than from the smaller, single-product business of a decade ago. Owner earnings per share (cash generated by the business after removing working-capital swings, stock-based pay, and the portion of capital spending needed just to keep the existing business running) were roughly $0.48 in 2023, $0.95 in 2024 and $1.38 in 2025. We used the middle of those three years, about $0.95 per share, as the normal starting level, and excluded 2022 because that year's result was distorted by an $11.9 million one-time non-cash charge tied to the AffloVest earn-out. Both product lines have been growing at a healthy clip: lymphedema revenue was up 7% and airway-clearance revenue up 52% in 2025, with Medicare's share of revenue rising from 18% to 24%. That is faster than we think is sustainable for a decade, so our base case starts year one at 12% growth - in line with 2025's overall revenue growth - and gradually fades to roughly 5% by year ten as the business matures. Beyond year ten, we assume owner earnings settle at a lower, sustained plateau (about 85% of the year-ten level, held flat) rather than continuing to compound, since compression and airway-clearance devices operate in a competitive field with several established rivals and no patent-protected exclusivity. The most important thing to watch is reimbursement. Starting in April 2026, Medicare began requiring prior authorization before it will pay for pneumatic compression devices - the company's largest product category - and Medicare has just become a bigger share of the business. Our bear case assumes this new process causes real friction: claim denials and processing delays that stall growth and even reduce owner earnings for a couple of years before the business works through the backlog and finds its footing, settling into a smaller, permanently more competitive market. Early cash-flow data from the first half of 2026 already shows a slowdown consistent with this kind of disruption, though it is too soon to know if it is temporary or lasting. In this case we estimate the shares are worth around $10 each. Our bull case assumes the new authorization process turns out to be a manageable administrative step (the company already runs prior-authorization processes for its private-insurance business) and that both product lines keep gaining share, supporting a value closer to $46 per share. On balance we estimate the shares are worth about $27 today, with a meaningful chance that reimbursement friction from the new Medicare requirement proves worse than expected. Given that risk, the margin-of-safety price sits meaningfully below our estimate of value - around $15 per share - to leave room for the reimbursement transition to go poorly. The balance sheet is conservative (low borrowings, no near-term debt maturities of concern), and the company has no material ongoing legal or accounting issues that we could find in its filings.
