Electromed makes the SmartVest System, a high-frequency chest wall oscillation (HFCWO) vest that helps patients with bronchiectasis and other chronic lung conditions clear mucus at home. In fiscal 2026 (year ended June 30, 2026) it had revenue of $73.8 million, up 15.3%. Homecare was $66.6 million of that; hospitals were $3.4 million, and sales through home-medical-equipment distributors were $3.3 million. Of the $73.4 million of U.S. revenue, about 95% was homecare. Gross margin was 78.5%, operating income was $13.9 million and net income was $11.3 million. It has no borrowings on a $10 million bank credit line, which matures in December 2026 unless renewed. How we measure earnings. We start from operating cash flow, take out the swing in working capital, and deduct stock-based compensation ($2.7 million in fiscal 2026) as a real cost. We deduct only the capital spending needed to stand still, not the part that supports growth. On this basis owner earnings per share were $0.29, $0.25, $0.16, $0.45, $0.28, $0.13, $0.26, $0.60, $0.96 and $1.35 for fiscal 2017 through 2026. Fiscal 2026 is flattered by removing a $5.1 million build in receivables from operating cash flow; operating cash flow actually fell to $9.7 million from $11.4 million even though net income rose 50%. Receivables grew more slowly than revenue, but we could not tell from the filing summary whether collection is slowing. That is worth watching. Starting point. The business earned far less per share before fiscal 2024 (about $0.13 to $0.45), when revenue was under $50 million. Since then revenue has risen to $55 million, $64 million and $74 million, so we use the median of fiscal 2024 to 2026, $0.96 per share, as today's normal level. That is below the fiscal 2026 figure of $1.35, on purpose, because one year of a rapid step-up is not enough to move the level. Owner earnings grew 41% in fiscal 2026 from the year before; the base case assumes only 10% in its first year from the $0.96 starting point. What growth we counted. Revenue has grown roughly 14% to 17% in each of the last five fiscal years, mainly by adding field sales representatives (an average of 58 in fiscal 2026 versus 54 in fiscal 2025) and getting more approvals per representative. That has been funded from the company's own cash and continued through the pandemic years. We credit that engine at a rate well below its record, tapering as the room narrows. We do not credit margin gains, new products, share gains from competitors Baxter (Hill-Rom) or Philips, or any expansion of the patient market, because none is contracted or already done. Amortization of intangibles is small ($208,000), so no wasting-asset adjustment mattered. Base case: $16.80 per share. Owner earnings grow from $0.96 by 10% in the first year, tapering by one point a year to 1% in year ten (reaching about $1.63), then hold flat for ten more years, reflecting the established reimbursement code for HFCWO devices (Medicare allows roughly $16,000 for the SmartVest under code E0483) and a product line the company has sold for over two decades. After that we assume nothing, because nothing we can name and evidence continues beyond it. Bear case: $3.30 per share. Most of the company's revenue is homecare paid by Medicare, Medicaid, commercial insurers and the VA, and the filing describes budget pressure on federal health programs and growing price sensitivity among payers. The bear case assumes growth stops and reimbursement and selling-cost pressure cut owner earnings back to about $0.45 per share, the level reached in fiscal 2020 on roughly half today's revenue, held for ten years and then nothing. Debt is not a factor since none is drawn. Under this reading the shares are still worth more than nothing, though the cushion is thin. Bull case: $27.10 per share. The sales-force engine keeps working as it has, starting from the fiscal 2026 level of $1.35 per share, growing 12% in the first year and tapering by one point a year to 3% in year ten (about $2.77), then holding flat for ten years. Cash. Cash of $20.5 million at June 30, 2026 is not added on top of these values, because owner earnings already capture the cash the business produces and adding the balance would count it twice. On this basis the value of each case comes entirely from future owner earnings. What could make this wrong. The value depends on one product category paid largely by government programs; a reimbursement cut would hit earnings quickly given the high fixed selling cost base. The company also sells its main product through a small number of distributors and depends on continued sales-force expansion. On the other side, the company is not subject to Medicare competitive bidding, which supports pricing. Margin-of-safety price. The margin-of-safety price is about $9.00 per share, roughly 46% below the base value, a wide margin because reimbursement is set by government payers, the company is small and the bear case sits far below the base. From that price the base case implies about a 20% annual return if it plays out.
