Omnicell sells medication-management automation (dispensing cabinets, pharmacy robotics, medication packaging) and related software and services to hospitals and pharmacies, almost entirely in the United States (about $1,065 million of its $1,185 million 2025 revenue). Sales are lumpy because hospitals buy capital equipment in bursts, but a layer of service and subscription contracts sits underneath: remaining contracted revenue was $375 million at June 30, 2026 ($388 million at year-end 2025, $392 million at year-end 2024). What the business earns. We measure owner earnings as operating cash flow, less the swings in working capital (receivables, inventory, payables, deferred revenue), less stock compensation, less the capital spending needed to stay in business, which at Omnicell includes the $13-46 million a year it capitalizes on software it sells. Stock compensation is a real cost here ($68 million in 2022, $45 million in 2025) and working capital has flattered reported cash flow: operating cash flow of $181 million in 2023 and $188 million in 2024 included working-capital releases of $56 million and $79 million, which we remove. Before lenders are paid, this leaves business cash flow per share of about $1.60 (2019), $0.66 (2020), $2.17 (2021), $0.26 (2022), $0.35 (2023), $0.40 (2024) and $0.54 (2025). Net income was only $2 million in 2025 and operating income $5 million; cash earnings are higher than accounting profit because $79 million of depreciation and amortization is non-cash, but about $58 million of capital spending (including capitalized software) must be replaced each year. We use the median of the seven years 2019-2025 (a full run-up, pandemic boom, destocking trough and three weak years): $0.54 per share, about $25 million a year. We did not use the first half of 2026 to lift this level: operating income was $49 million versus a loss of $3 million a year earlier, and the gross margin rose to 47.1% from 42.6% on revenue up 11% to $622 million, but that is two quarters, and we could not separate any one-time items (the company's 10-K flags possible refunds of tariffs struck down by the Supreme Court on February 20, 2026). If the improvement holds it appears in the bull case, not the base. Growth check. Business cash flow per share rose from $0.40 in 2024 to $0.54 in 2025 (+35%, from a very small base). The base case assumes 0% growth in year 1, below that, because contracted revenue is drifting down rather than up and nothing in the filings dates a reversal. Debt and cash. Omnicell owes $172.5 million of 1.00% convertible notes due December 1, 2029 (conversion price about $57.25, so we treat them as debt to be repaid in cash). It repaid the remaining $175 million of its older notes in September 2025, has a $350 million credit line expiring October 2028 with nothing drawn at year-end 2025, and held $292 million of cash at June 30, 2026. We treat $100 million as needed to run the business and the rest as excess. Because the notes cost 1% against an ordinary rate several points higher for roughly 3.4 more years, we value them at about $147 million. Net of excess cash, the company holds roughly $46 million more than it owes, about $0.99 per share on 46.1 million diluted shares. This is based on the year-end 2025 filing for the notes and the June 2026 filing for cash. The diluted share count has held near 46 million despite stock compensation, helped by buybacks ($77.6 million in 2025, none in the first half of 2026). Bear case: $4.04 per share. Hospital capital spending stalls and Omnicell repeats its own worst recent stretch: three years at the 2022 level of $0.26 per share (2022 operating cash flow fell to $78 million after a $53 million working-capital outflow and $68 million of stock compensation), then only the 2023 level of $0.35 through year 10, as contracted revenue keeps slipping and hospital consolidation brings pricing pressure noted in the risk factors. Beyond year 10 earnings fade to zero over ten years as the installed base is not replaced. The business is worth about $141 million ($3.05 per share); net of excess cash it owes nothing, adding $0.99. Cash flow of roughly $12 million a year still covers the roughly $2 million annual interest, and cash exceeds the 2029 notes. Base case: $7.06 per share. Business cash flow stays at $0.54 per share for fifteen years, then fades to zero over the next ten. Evidence: revenue has stayed between $1.11 billion and $1.30 billion for five years (2021-2025) and $375 million of revenue is contracted. No growth is credited: revenue growth has been uneven (up 14% in 2022, down 11% in 2023) rather than a steady repeatable mechanism, and margin gains and new products are not secured. The business is worth about $280 million ($6.07 per share), with net cash adding $0.99. Bull case: $23.47 per share. The first-half 2026 gross margin improvement is sustained and cash flow returns to $2.00 per share, between the levels actually achieved in 2019 ($1.60) and 2021 ($2.17), held fifteen years and then faded over ten. The business is worth about $1,037 million ($22.48 per share), plus $0.99 of net cash. What would make this wrong. If the 2026 margin gains prove permanent, the base is too low and the bull is closer to right; if they reflect one-time items, the weak 2022-2024 pattern returns. Our earlier write-up described the company as carrying little debt; it has the $172.5 million of convertible notes described above, and its figures were built on a different basis and are not comparable. Margin-of-safety price: $3.50. We are only moderately sure of the earnings level: business cash flow has ranged from $0.26 to $2.17 per share in seven years and reported profit was small or negative in 2023-2025. The worst plausible outcome is another multi-year ordering freeze with high stock compensation; the balance sheet survives it (cash exceeds the notes, the credit line was undrawn at year-end 2025, the first maturity is December 2029) but earnings could stay thin for years. The price assumes a holding period of about three years; during that period, about $1 of net cash per share and buybacks support the shares. At $3.50, about $1 is covered by net cash, leaving the operating business priced at roughly $2.50, a cash yield of about 21% on the 2025 level. From that price the base path implies roughly 21% a year, and the bear path about 9% a year, so the price is not set higher.

Sales geography
United States (about 90% of revenue, nationwide hospitals and pharmacies); rest of world about 10%
Historical growth strategy
Grew by selling automated medication-management hardware into hospitals and then layering software, service and subscription contracts on the installed base, supplemented by acquisitions; revenue rose from about $696 million in 2016 to $1.18 billion in 2025 with uneven year-to-year results.
Chart analysis
The shares closed at $35.21 on October 7, 2026, in the lower half of a 52-week range of $29.06 to $55.00. Since late August 2026 the stock has been range-bound roughly between $32 and $35.6, a consolidation after a decline from the 52-week high. A chartist would call $29 the key support and the mid-$30s the near-term resistance, and would want a close above that band, or a rise toward the $40s, before calling the downtrend over; a break below $29 would open further downside.
