MediciNova is a clinical-stage drug developer with no product on the market. Its lead candidate, MN-166 (ibudilast), is in a Phase 2b/3 trial in ALS (COMBAT-ALS), in which 234 patients have been randomized across two arms in the United States and Canada; enrollment finished in 2025 and results have not yet been reported. The company also holds rights to MN-001 (tipelukast), licensed from Kyorin. Its only revenue, about $0.4 million in 2025 and $0.65 million in the first half of 2026, is payment for research services under an agreement with the Mayo Foundation for Medical Education and Research, whose term runs to August 2026. The business loses money every year and has not generated owner earnings in any year we measured. Cash earnings after stock compensation and capital spending were about -$10.1 million in 2021, -$14.1 million in 2022, -$8.6 million in 2023, -$11.0 million in 2024 and -$12.0 million in 2025, roughly 49 million diluted shares each year. There is no earnings stream to project. We found no borrowings, so there is nothing owed to lenders other than a small office lease and ordinary payables. The company has never paid a dividend and says it does not expect to. Dilution has been slight: diluted shares rose from 48.6 million in 2021 to 49.2 million in 2026. What the shares are worth today therefore comes from the cash on hand, less what it costs to reach the trial result. Cash was $30.8 million at the end of 2025 and $25.4 million at June 30, 2026; operating cash use in the first half of 2026 was $5.4 million. Management says it expects 2026 operating cash needs of about $16.2 million and assumes similar spending in 2027, and believes current funds cover at least twelve months. Base case, $0.36 per share: the trial result is not counted as a source of value. Success is not secured, and no approval, partner or sales price for MN-166 in ALS is contracted. Cash falls to roughly $19.9 million by year-end 2026 (the first-half pace repeated), about $2 million is set aside for closing out studies, the lease and severance, and the remaining roughly $17.9 million is about $0.36 per share. The business is worth about that, owes essentially nothing, and the difference is the same $0.36. The ALS patent covering use of ibudilast expires no earlier than January 2029 and the ALS combination-with-riluzole patent no earlier than November 2035, so a successful result would not be without protection, but that is not credited. Bear case, $0.06 per share: the ALS trial fails and the company continues to spend at roughly $13 million a year through 2027, between the first-half 2026 pace and management's $16.2 million estimate, on its other candidates. Cash of $30.8 million at the end of 2025 would fall to about $4.8 million, less about $2 million to wind down, leaving about $2.8 million or $0.06 per share. The ALS field has a long record of failed trials, and an ALS-focused company with one lead asset has no other revenue to absorb a failure. Bull case, $0.40 per share: the trial succeeds and the company keeps its cash intact, with no further dilution before year-end 2026, so about $19.9 million, or $0.40 per share, remains. We do not credit any value for approval, partnering, or the multiple sclerosis and degenerative cervical myelopathy programs, because none of these has a contract, price or secured funding behind it. This is a deliberate limitation: if the trial succeeds, the shares could be worth much more than this case shows, but we cannot name an evidenced amount. Likewise a failure is only partly captured by the bear case. What would make this wrong: a positive result paired with a partnership or an approval pathway would be worth far more than any case here; a negative result followed by a rapid cost cut could leave more cash than the bear case shows. The company's own filing warns that it will need additional capital to complete development and commercialization of any product. Analyses before this one gave much higher values by weighing the chance of trial success; those figures are not comparable with the ones here, which count only cash and contracted items. Margin-of-safety price: we are sure of the cash balance because it is reported, and the operating burn has been steady at $9 to $14 million a year for five years. We are not sure of the trial result, which cannot be forecast. The worst plausible outcome is a failed trial followed by continued spending on weaker candidates, leaving a few million dollars of cash; the structure survives that, since there is no debt, but little would remain for shareholders. If we are wrong about the pace of spending or the wind-down costs, the shares are worth between about $0.06 and $0.40. The price assumes a holding period through the trial result, roughly the next several months to a year, and the business pays nothing in the meantime and has never paid a dividend. At the margin-of-safety price of $0.30, the shares represent a claim on most of the remaining cash, with the trial result as additional value that no case credits; below that cash cushion, the 20% annual return target does not depend on the trial succeeding.

Sales geography
United States (the only revenue, about $0.4 million in 2025, is research-services revenue from the Mayo Foundation for Medical Education and Research); no product sales anywhere. Company is a US-listed developer with its drug licensing roots in Japan.
Historical growth strategy
In-licenses drug candidates from Japanese pharmaceutical companies (for example ibudilast and tipelukast), runs small clinical trials funded by equity raises and keeps a very small staff; growth has come from advancing trials and seeking partners rather than from sales or acquisitions.
Chart analysis
A chartist would treat this as a low-priced clinical-stage biotech whose chart is driven by trial news rather than by trend. Such stocks usually drift in a range while a binary trial result is pending, with sharp gaps in either direction when data arrive. A chartist would look for a high-volume break above the top of the multi-month range as confirmation of a new uptrend, and treat a break below the range lows as a sign of a weakening trend. Volume spikes and the position relative to the 50-day and 200-day averages would be the signals to watch; with no earnings to anchor it, technical levels are weak support and resistance.
