Intuitive Surgical designs, manufactures, and sells the da Vinci family of robotic-assisted surgical systems and the Ion platform for minimally invasive lung biopsies. Most of the company's profit does not come from selling the surgical systems themselves, which are large capital purchases hospitals make only occasionally, but from the steady stream of specialized instruments, accessories, and service contracts that customers buy every time a system is used. That recurring, high-margin revenue, tied to a large and growing installed base of systems worldwide, is the core of the company's value. To estimate what the shares are worth, we started from the cash the business actually generates for shareholders after covering the ongoing cost of running and maintaining its current operations - what we call owner earnings. Two accounting judgments mattered here. First, the company has been spending heavily on new manufacturing and distribution facilities over the past few years (capital spending roughly doubled in 2023 and 2024), explicitly to build capacity for future growth rather than to sustain today's business; we treated that expansion spending as investment in future growth, not a current operating cost, using the company's more typical pre-expansion spending level as the baseline for ongoing facility needs. Second, the company has been building inventory rapidly to support its newest system launch and strong volume growth; we treated that as a swing in working capital rather than a permanent addition to distributable cash, which is a conservative treatment that understates earnings power in a year of especially rapid inventory growth, like the most recently completed one. Looking at the business over several recent years at its current scale, we estimate normalized owner earnings of roughly $4.55 per share as our starting point, well below the more than $8 per share the most recent year alone would suggest, because that year's figure was inflated by an unusually large inventory build. We modeled three scenarios over the next ten years, plus a view of what follows. Our bear case assumes competitors - notably Medtronic and Johnson & Johnson, both of which have disclosed robotic-assisted surgery products aimed at Intuitive's core markets - take meaningful share over the back half of this decade, procedure growth slows toward the low single digits, and margins compress modestly as the company works to defend its position. Earnings per share grow only slightly before flattening. We do not assume lasting damage to the underlying business, given its lack of debt and decades-long surgeon and hospital relationships. Our base case assumes continued growth at a decelerating pace as the business matures - roughly low-double-digit growth in the next few years, tapering to mid-single digits by year ten - reflecting continued adoption of the newest system generation and the Ion platform, without assuming the exceptional growth of the past couple of years persists indefinitely. Our bull case assumes the newest system generation and the lung-biopsy platform gain adoption faster than the base case expects, supported by continued international expansion and efficiency gains from the recent facility investments, sustaining double-digit growth for longer before gradually slowing. In all three cases we treated the years beyond the ten-year forecast conservatively: because today's products depend on continued successful innovation rather than protection that simply renews itself, we assumed only a modest, low-single-digit sustained growth rate beyond year ten. That long-run component makes up a significant share of our estimated value in every case, which is typical for a business with this kind of durable but not indefinitely guaranteed competitive position - so it is the assumption most worth revisiting as new information arrives. Putting the cases together, we estimate the shares are worth roughly $76 in a difficult scenario, $172 in our central scenario, and $233 in a favorable scenario. Because even the downside case remains solidly positive - reflecting the company's lack of debt and highly recurring revenue base - the margin-of-safety price sits considerably below our central estimate, at around $106 per share, to allow for the risk that competition or slower adoption proves more damaging than we currently expect.
