Business condition attached to the margin of safety The loss-narrowing trend already visible in Q2 2026 (net loss of ~$13M, down from ~$322M a year earlier) is confirmed by at least two more quarters of similar improvement, with Brinsupri's US revenue ramp and gross-margin gains continuing through late 2026/early 2027 rather than stalling after the initial patient pull-through.
Insmed is a commercial-stage biopharmaceutical company built around therapies for serious, often fatal lung disease. Its lead product, ARIKAYCE, has treated a rare lung infection since 2018 and grew revenue 19% in 2025, led by a 41% jump in sales outside the US. In August 2025 the company won FDA approval (and in November 2025, EU approval) for a second medicine, Brinsupri (brensocatib), the first approved treatment for non-cystic fibrosis bronchiectasis, a chronic lung disease the company estimates affects roughly 500,000 diagnosed patients in the US, 600,000 across the five largest European markets, and 150,000 in Japan. Brinsupri's launch has been unusually fast: it generated $173 million of US sales in its first five months on the market, and combined company revenue in the first half of 2026 ($731 million) already exceeds all of 2025 ($606 million). A third program, an inhaled therapy for pulmonary hypertension called TPIP, is entering Phase 3 testing in 2026 after encouraging mid-stage results, but has not yet produced pivotal data, so we have not given it value in our base or bear cases. Funding two drug launches and a growing pipeline has been expensive: cash used in operations rose every year from 2018 through 2025, reaching $935 million last year, as the company built out sales forces for two products and scaled manufacturing and R&D. That said, the trend has already turned -- net losses narrowed sharply in the first half of 2026, and the second quarter alone was nearly break-even, as Brinsupri revenue scaled and cost of goods fell (helped by Brinsupri's lower manufacturing cost versus ARIKAYCE). We used 2025 as the year that best represents the underlying, launch-stage cash economics of the business today, and built our estimate of what it can sustainably generate for shareholders from there, stripping out one-time working-capital swings and treating stock-based compensation -- which is large and growing -- as a real cost rather than an accounting add-back. The base case assumes the improvement already visible in 2026 continues: losses keep narrowing over the next two to three years as Brinsupri approaches a meaningful share of its addressable patient population and ARIKAYCE keeps growing internationally, with the business turning cash-generative within a few years and settling into a mature, profitable specialty-pharmaceutical margin over the following decade. On that basis, we estimate the shares are worth roughly $40 today. The bear case assumes the launch decelerates well before Brinsupri reaches a meaningful share of its market -- a common pattern for newly launched specialty drugs once the most eager patients have started treatment -- while two real financial obligations continue regardless: a $500 million term loan, a royalty obligation equal to 4% of ARIKAYCE's global sales, and up to $326 million of milestone payments owed to the founders of a company Insmed acquired in 2023, a large share of which the company now considers probable given recent pipeline progress. Insmed has also funded itself heavily through new stock issuance (over $1.9 billion combined in 2024 and 2025), diluting shareholders by roughly a third in eighteen months; a slower path to profitability would likely mean more of the same. After covering those obligations and further dilution, we estimate little value would be left for shareholders in this scenario -- around $1 per share. The bull case assumes Brinsupri's launch continues at roughly the pace already shown in the first half of 2026, both products expand their share of a still-underpenetrated global patient population (Brinsupri's European launch only began in November 2025), and TPIP's already-reported positive mid-stage pulmonary hypertension results translate into an approved product later in the decade. On that basis, we estimate the shares could be worth roughly $96. Because so much of the value here depends on a launch that is only a few quarters old, the margin-of-safety price of near $20 is set with the risk of a disappointing outcome in view, and assumes the recent improvement holds for a couple more quarters.
