United Therapeutics sells treatments for pulmonary arterial hypertension (PAH) and related lung disease, almost all built on one molecule, treprostinil. In 2025 revenue was $3.18 billion: Tyvaso DPI (inhaled dry powder) $1.29 billion, nebulized Tyvaso $586 million, Remodulin (infused) $527 million, Orenitram (oral tablet) $497 million, the cancer drug Unituxin $227 million, and small amounts of Adcirca and other items. About 95% of sales are in the United States. The company has no debt and held $3.80 billion of cash and marketable investments at June 30, 2026, after spending $1.0 billion on share buybacks in 2025 and another $1.5 billion in the first half of 2026. What the business earns. We measure the cash the business produces after the capital spending needed to keep today's products going, and we treat stock compensation as a real cost. On that basis, earnings grew strongly from $792 million in 2023 to $975 million in 2024 and $1.27 billion in 2025. For valuing the operations we then strip out the interest earned on the company's large investment portfolio (about $150 million a year after tax), because we count that cash separately below. Three accounting judgements matter. First, capital spending jumped to $521 million in 2025 because the company is building a new manufacturing plant in Research Triangle Park and facilities in Minnesota and Texas (adding to one in Virginia) to raise pigs for its experimental organ-transplant program; we treated only spending in line with the company's ten-year average (about 7.8% of sales) as the ongoing cost of the current business. Second, we included a working-capital line in the cash-flow statement that our earlier figures had missed; this raised some past years (2025 by about $81 million) and lowered others. Third, 2025 cash taxes were unusually low because a July 2025 tax law restored immediate deduction of research spending and let the company catch up on earlier years, so we did not treat 2025 as the normal level. The starting point. We use the middle of the last three years (2023 to 2025, the period since Tyvaso DPI became the main product), which is about $860 million a year for the operations. That matches what the first half of 2026 shows: total revenue fell 2% from a year earlier and operating profit fell about 12% (down 15% in the first quarter and 9% in the second), as nebulized Tyvaso (down 18% in the second quarter) and Remodulin (down 6%) lost ground and Tyvaso DPI growth slowed to 4%. The company itself attributes the weakness to competing therapies. Owner earnings grew about 32% per share in 2025, but because two consecutive quarters now show decline and 2025 was flattered by the tax catch-up, every case starts at roughly $870 million a year for 2026 and no case assumes growth from that level. The pressures are dated and named in the company's own filings. Liquidia's Yutrepia, an inhaled treprostinil powder approved in May 2025 for both PAH and PH-ILD, has been competing directly with Tyvaso since June 2025. Under past settlements, a generic nebulized Tyvaso may launch as soon as it wins FDA approval (permitted since January 2026), and generic Orenitram is allowed from June 2027 and December 2027. Merck's Winrevair is a newer PAH therapy that can delay or reduce use of treprostinil products. The company is asking a court to force Liquidia to remove PH-ILD from Yutrepia's label under a patent running to February 2042; the trial took place in June 2025 and, as of the latest annual report, no decision had been issued. Base case (about $240 per share). Tyvaso DPI holds its current sales level of about $1.3 billion, nebulized Tyvaso keeps shrinking about 12% a year, Remodulin 5% a year (close to its recent pace, years after its own generic arrived in 2019), and Orenitram loses about 70% of its sales by 2029 after generics arrive. About 40% of costs, mainly commercial spending, shrink with sales. Cash earnings fall from about $870 million to about $550 million by 2030, a drop of roughly one-third, very similar to what happened in 2017 to 2020 when Adcirca and Remodulin faced generics. From there, the company's research engine, which has produced a new treprostinil product or label expansion every few years for two decades (Tyvaso 2009, Orenitram 2013, the PH-ILD label in 2021, Tyvaso DPI in 2022) and whose roughly $550 million annual cost is already deducted, holds earnings at that level through 2045, after which we fade them to zero by 2055. The operations are worth about $7.75 billion; the company owes nothing, and we add $3.24 billion of surplus cash (the $3.80 billion balance less about $313 million still budgeted for the new facilities and $250 million kept for operations), for $10.99 billion, or about $240 per share on 45.8 million diluted shares. Bear case (about $168 per share). Yutrepia and Winrevair take share so that Tyvaso DPI falls about 4% a year to 2030 and then 5% a year, nebulized Tyvaso falls 18% a year, Remodulin 7% a year, and Orenitram loses over 80% of its sales. Cash earnings fall to about $330 million by 2031 and then keep slipping 3% a year as the research engine fails to fully replace erosion, ending by 2050. The operations are worth about $4.43 billion; with no debt and $3.24 billion of surplus cash, shareholders' value is about $7.67 billion, or about $168 per share. Because the company has no borrowings, even this case carries no refinancing risk; the cash alone is worth about $71 per share. Bull case (about $279 per share). The company wins the PH-ILD patent case, keeping Yutrepia out of PH-ILD and protecting Tyvaso DPI; Orenitram erodes more slowly (about half of its sales lost), as Remodulin's generic erosion was mild; nebulized Tyvaso declines 8% a year and Remodulin 3%. Cash earnings bottom at about $645 million in 2030 and hold through 2050 before fading to zero by 2060. The operations are worth about $9.53 billion; with no debt and $3.24 billion of surplus cash, about $12.77 billion, or about $279 per share. What we left out, and why. None of the cases credits the idiopathic pulmonary fibrosis (IPF) opportunity, even though both large trials of nebulized Tyvaso in IPF (TETON-2 in September 2025 and TETON-1 in March 2026) met their goals and the company plans to seek approval; until the FDA approves it and sales appear, it is a hope rather than an earning asset. The same applies to the progressive pulmonary fibrosis trial, the ralinepag program and the pig-organ transplant program. If IPF is approved and adopted, the shares could be worth considerably more than any of our cases; that is the main way this analysis could prove too cautious. It could prove too generous if Orenitram generics arrive earlier than the settlement dates, if generic nebulized Tyvaso wins approval and takes share quickly, or if the recent Medicaid funding cuts squeeze pricing. Change since our previous estimate. We previously valued the shares at $458 to $1,281 with a base of $884. That estimate assumed the recent growth would slow gradually but continue for many years. The first half of 2026 shows sales and operating profit already falling, and the generic dates for nebulized Tyvaso and Orenitram are fixed, so we now model a decline to a lower, sustained level instead of continued growth. We also now value the operations excluding investment income and count the company's cash separately and explicitly (adding about $71 per share), and the company has no debt, the last $300 million of bank borrowing having been repaid in 2025. The net effect is a much lower estimate.

Chart analysis
The shares trade around $475, below both the 50-day moving average (about $511) and the 200-day moving average (about $540), with the 50-day now under the 200-day, a bearish crossover that chartists read as a shift from uptrend to downtrend. The stock is roughly 22% off its 52-week high near $609 and about 16% above its 52-week low near $409, so it sits in the lower-middle of its range. The longer multi-year uptrend from the low $200s remains intact on a long-term chart, but the pattern of lower highs since the peak suggests a correction phase; a chartist would watch the $409 low as support and a reclaim of the 200-day average as the signal that the uptrend has resumed.
