Viemed Healthcare rents and services in-home breathing equipment, mainly non-invasive ventilators for patients with chronic respiratory failure from COPD, plus sleep and oxygen/other respiratory products. Revenue grew from $80M in 2019 to $270M in 2025. Monthly ventilator rentals were about 51% of 2025 revenue (56% in 2024); Medicare and Medicaid were about 40% of revenue. Why earnings power is much smaller than reported profit. The company reported net income of $14.9M in 2025 and operating cash flow of $51.9M, but it spends $40M a year on equipment (mostly the rental ventilator fleet, which wears out and is replaced). After removing swings in receivables and similar timing items, deducting stock compensation ($9.1M in 2025), and separating growth spending from replacement spending using the company's own 2018-2025 spending pattern (about 14% of revenue), we estimate real free earnings of roughly $12M in 2025, or about $0.29 per share. The series is uneven: $0.63 (2020, a pandemic ventilator surge), $0.11 (2021), $0.07 (2022), $0.29 (2023), $0.02 (2024, heavy fleet spending), $0.29 (2025). We use 2022-2025 as the reference window because 2020-2021 were distorted by the pandemic, and take the middle of that window, about $0.18 per share. Bad debts were treated as a real cost. We did not adjust the level for the fact that depreciation ($28.6M) is below total equipment spending; a lower replacement estimate would raise our figures, and this is the main judgement in the numbers. Base case (about $3.79 per share): $0.18 a year, growing 3% a year for ten years and then held flat. That growth reflects a modest continuation of steady patient additions; the company has grown revenue in every year but 2021. We do not credit the very fast recent revenue growth, share gains or new product lines because they are not locked in. Year-one growth is set at 3%, versus 2025 actual earnings growth that is not meaningful given the swing from $0.015 to $0.29; the assumption sits below the two-year average of the recent range. Bear case (about $0.43): In June 2025 Medicare finalized a national coverage decision that sets specific medical-necessity criteria for home ventilators for COPD, and the filing says it may materially affect access, reimbursement and utilization, and could carry into Medicare Advantage and commercial plans. Ventilator rentals are over half of revenue. The bear case has earnings at the 2024 trough level of about $0.05 and shrinking 5% a year. Future Medicare competitive bidding is a further disclosed risk, although the company says the next round is not expected to apply to its products. Bull case (about $7.15): Earnings hold at the $0.29 level reached in both 2023 and 2025 and grow 5% a year for ten years as the fleet keeps expanding at the pace of the last several years, then hold flat. What could make this wrong: the coverage decision proving milder than feared would favor the bull case; heavier payer pushback on ventilator rentals would favor the bear case. Equipment spending intensity is also unsettled: if replacement needs run closer to depreciation, values are higher. The company carries no going-concern warning and is listed on Nasdaq. The valuation is the value of the equity directly; debt is not subtracted separately because interest is already in the cash flows.
