Trevi Therapeutics, Inc. (TRVI) stock intrinsic value fair value estimate & owner earnings analysis

Margin of safety$0.00 ComplexityAverage Updated
Business condition attached to the margin of safety Haduvio shows positive results in its first Phase 3 trial for IPF-related chronic cough and the company has a credible path to FDA approval.
Trevi Therapeutics is a clinical-stage biopharmaceutical company with one product candidate, Haduvio (oral nalbuphine extended-release), being developed for chronic cough in patients with idiopathic pulmonary fibrosis (IPF), chronic cough in other interstitial lung disease, and refractory chronic cough. The company has never had revenue from product sales and has lost money every year since it began operations in 2011; its accumulated deficit was $329.8 million at the end of 2025. Owner earnings have been negative in every year we could measure. Adjusting operating cash flow for working-capital timing and stock compensation, and with almost no capital spending, the company consumed roughly $31 million to $47 million a year from 2021 to 2025. Per share that is about -$0.35 in 2025 (-$0.43 in 2024, -$0.36 in 2023, -$0.47 in 2022). The 2021 figure of -$1.41 per share reflects a much smaller share count before later share issuances. Because the company earns nothing today, there is no earnings level to build a value on. What exists that we can point to: Haduvio produced positive results in two mid-stage trials (the Phase 2b CORAL trial in IPF cough and the Phase 2a RIVER trial in refractory chronic cough, announced March 2025), and the company has completed an End-of-Phase 2 meeting with the FDA and expects to start two Phase 3 trials in the second quarter of 2026. What does not yet exist: a successful Phase 3 trial, an FDA approval, a partner, or a commercial product. The filing itself says earlier positive results may not predict Phase 3 results, that the FDA's approach to some trial endpoints is unclear, and that nalbuphine carries an opioid-class label warning that could restrict how the drug is marketed. Under our approach we credit only value that rests on things already in place, so none of this future potential is included in any case. Bear case: $0 per share. Phase 3 fails or approval is denied and there is no other product. Base case: $0 per share. No approval, sales or contract exists today, so nothing evidenced produces future owner earnings, and continued research spending is a cost rather than a source of value. Bull case: $0 per share. Even if the Phase 3 trials go as planned, positive results and approval are events that have not happened yet, so they are not counted. Cash held on the balance sheet is also not added, because the company is spending it on trials and is expected to keep doing so: at December 31, 2025 it held $188.3 million of cash, cash equivalents and marketable securities, which management believes funds operations into 2028, and it reported cash and equivalents of $54.3 million at June 30, 2026 in its latest quarterly filing. A zero valuation here means we cannot support any value from evidence available today; it does not mean the shares cannot be worth something if the drug is approved. That outcome would be a very different company from the one in the filings, and would need to be re-valued once Phase 3 data exist. The main things that would prove this view too cautious are strong Phase 3 results in IPF-related chronic cough, FDA acceptance of an application, or a licensing or commercial agreement with committed payments. Any value in the shares today rests on a clinical outcome, not on earnings.
