Rhythm Pharmaceuticals sells IMCIVREE (setmelanotide), a daily injection for rare genetic and brain-injury forms of severe obesity. It is approved for obesity caused by POMC, PCSK1 or LEPR deficiency and by Bardet-Biedl syndrome, and, according to press reports, on March 19, 2026 the FDA added acquired hypothalamic obesity (obesity following damage to the hypothalamus) for patients 4 years and older. Net product revenue has grown every year since the March 2021 U.S. launch, from $3.2 million in 2021 to $194.8 million in 2025, and was $131.4 million in the first half of 2026 versus $81.2 million a year earlier. About 90% of each sales dollar is left after the cost of the product (2025). The problem for a value estimate is that the company has never produced cash for its owners. Operating cash flow was negative in every year from 2017 to 2025 (a loss of $115.7 million in 2025), and stock compensation, which we count as a real cost, was another $66.8 million. On our measure the business consumed about $2.80 per share in 2025, versus $2.56 in 2024, so owner earnings did not improve last year. Over nine years of filings the figure ranges from about minus $2.25 to minus $3.90 per share. There is no positive earnings level to build a value on. We tested whether the business as it stands could earn money if it stopped investing for growth. Over the twelve months to June 2026, gross profit was roughly $218 million. The revenue-based payments owed to HealthCare Royalty (11.5% of the first $125 million of annual sales and 7.5% of sales between $125 million and $300 million) would take roughly $23 million of that, and selling and administrative spending was about $241 million. That leaves a loss of about $46 million before spending a dollar on research, and research spending was about $173 million. Reaching profit therefore depends on sales growing faster than costs. Growth like that is what the company hopes for, but we do not credit it because it depends on the new hypothalamic obesity launch, which management has described to investors as slow and steady, and on other sales that have not yet happened. The other side of the story is real and worth naming: the FDA has granted seven years of orphan-drug exclusivity for the drug's approved rare genetic uses, the main composition-of-matter patent has been extended in the United States through 2032, and the company reports 20 issued U.S. patents in its key families. If sales scale, the shares could be worth far more than we show. What the shares are worth on evidence alone is therefore the cash on hand, reduced by the money the business keeps using. At June 30, 2026 the company held about $331 million in cash and short-term investments, and it has said that its cash covers at least the next 24 months. Operating cash burn in the first half of 2026 was $53.3 million (versus $63.7 million a year before), and stock compensation was $49.2 million, so on our measure the business is using about $205 million a year. There are about 68.6 million shares, plus roughly 2.8 million more if the $131 million of convertible preferred stock converts, based on how the 17,500 preferred shares converted in 2025. Bear case, $0.00 per share: burn stays near the 2025 level of about $182 million a year, sales do not close the gap, and the preferred stock, which carries a liquidation preference and grows through paid-in-kind dividends, absorbs the remaining cash. Cash of $331 million less two years of that burn and the $131 million preferred claim leaves nothing for common holders, and any further funding would come from selling more shares. Base case, $0.00 per share: the business keeps using cash at the current pace of about $205 million a year. Two years of that exceeds the $331 million on hand. We give no value to sales growth or profit that has not yet appeared. Bull case, about $1.75 per share: the hypothalamic obesity launch works, sales rise enough that spending stops exceeding gross profit after about one year, and no further value is credited beyond that point. One year of burn leaves about $126 million, or $1.77 per share on about 71.4 million shares, which we round down to $1.75. We do not assign value to earnings after break-even because nothing in the filings lets us size them. Accounting notes: net income was adjusted for non-cash items in the usual way, and non-cash interest on the royalty financing agreement is added back inside operating cash flow. Only the payments that leave the company reduce cash. In 2024 a $92.4 million payment for acquired research assets (from LG Chem and Xinvento) was reported as an investing outflow, so it does not appear in operating cash flow. The 2025 revenue figure in the filing's geographic table ($194.8 million) differs slightly from another revenue figure in the electronic data ($189.8 million); we used the audited table. What would make this wrong: if hypothalamic obesity and existing indications together push annual sales well above the current pace while spending is held flat, the company could turn cash-positive sooner and the shares would be worth far more than we show. Nothing in the filings lets us size that outcome, so it is left out. This is the first analysis of this company.
