Precigen is now a one-product company. PAPZIMEOS (zopapogene imadenovec) got full FDA approval in August 2025 as the first and only approved treatment for adults with recurrent respiratory papillomatosis (RRP). RRP is a rare disease caused by chronic HPV 6/11 infection: growths keep coming back in the airway, and until now the only treatment was repeated surgery. PAPZIMEOS is given as four injections over 12 weeks. In its pivotal study, 51% of patients (18 of 35) needed no surgery in the 12 months after treatment, and 15 of those 18 were still in complete response at a median of 36 months. The company estimates about 27,000 US adults live with RRP. It makes the drug substance in its own facility in Germantown, Maryland. The rest of the company is small: a research-model business (Exemplar), an HPV cancer candidate (PRGN-2009) and earlier cell-therapy programs. History and how we measured earnings. Before the launch, Precigen, formerly Intrexon, lost cash every year. Owner earnings (operating cash flow, with working-capital swings, stock compensation and capital spending taken out) were between about -$59 million and -$202 million a year from 2016 to 2025, and about -$0.33 per share in each of 2023, 2024 and 2025. We do not use that record as the earnings level, because it describes a pre-commercial business that no longer exists. The 2025 reported net loss of $250.6 million overstates the cash loss. It includes a $139.5 million non-cash charge for the rising value of warrants issued with a December 2024 preferred-stock financing. That preferred converted to common stock in September 2025. We added the charge back, and our cash figures reconcile to the filed cash-flow statements. The launch year changes the picture. Revenue was $23 million in Q1 2026 and $55.0 million in Q2 2026, of which $53.1 million was PAPZIMEOS. Q2 operating income was $22.6 million, the company's first profitable quarter. Operating expenses in the quarter were $32.3 million, including about $7 million of R&D, so the product has very high gross margins. Management reported that more than 200 patients have received at least one dose, more than 100 have completed the course, and more than 500 have registered with the patient support hub. Private and government coverage now reaches nearly all insured US lives, and a permanent billing code took effect on April 1, 2026. Revenue is booked when the drug is delivered to the treating site, not when it is shipped into distributor inventory. One quarter of profit is not a track record, so we build each case from these facts rather than from the Q2 figure alone. Base case (about $2.10 per share). PAPZIMEOS revenue rises to about $250 million in 2027 as the registered-but-untreated backlog is worked through, and peaks near $270 million in 2028-2029. It then declines slowly to about $200 million by 2036. Two named reasons drive the decline. First, it is a one-time course with durable responses, so the pool of patients with the most frequent surgeries is gradually treated. Second, INOVIO's rival RRP product, INO-3107, is under FDA review with a decision due on October 30, 2026. We assume it is approved and takes part of new starts. Selling and administrative costs stay near today's roughly $90-100 million a year, and R&D near $32 million. Cash interest on the $100 million Pharmakon term loan (SOFR plus 6.5%, 3.75% floor) runs about $10 million a year until the loan amortizes between September 2028 and September 2030. About $1.12 billion of federal tax loss carryforwards keeps cash taxes minimal for roughly a decade. That yields owner earnings of about $100-117 million a year in 2027-2031. After 2036, revenue fades 10% a year for ten years and then ends. The US bars biosimilar copies for 12 years after approval, which runs to 2037. Beyond that, nothing we can evidence protects the franchise. Everything is discounted at 6% on about 414 million diluted shares. Bear case (about $0.45 per share). INO-3107 is approved and wins a large share of new patients. PAPZIMEOS peaks at today's run rate: about $180 million in 2027, falling 8-13% a year to about $60 million by 2036 as demand from the heaviest-burden patients is used up. After that it fades out entirely over five years. Costs are cut only with a lag. Owner earnings fall from about $43 million in 2027 to single digits by 2035. The balance sheet makes this case worse. Cash and investments were only $38.7 million at June 30, 2026, against $71.9 million of trade receivables, which the company expects to collect within about four months, and $100 million of debt that begins amortizing in 2028. If collections or sales disappoint, new shares or more borrowing (a further $25 million tranche is available until June 2027) would dilute holders. Bull case (about $4.26 per share). The company's own levers work: the hub backlog converts, the broad all-adults label pulls in patients with less frequent surgeries, and the European application filed in November 2025 is approved. INO-3107's device-based delivery limits its uptake. Revenue reaches about $400-430 million in 2028-2031 and then declines as the pool is penetrated. Even including European patients, that means treating the equivalent of only about a third of today's estimated US adult patient population over ten years. We gave no value to redosing, a pediatric label or PRGN-2009. Redosing is still in a trial, the pediatric trial has not started, and PRGN-2009 is still in trials. None has an approval or a demonstrated result to value. What could make this wrong. Everything rests on one product, just over a year into its launch, with a direct competitor weeks from an FDA decision. The key test is whether Q2's $53.1 million repeats and the large receivables balance turns into cash, as management says, reaching cash-flow break-even by the end of 2026. If Q3 and Q4 fall back toward the Q1 level, the base case is too high. If INO-3107 is rejected, or the backlog of more than 500 registered patients converts faster, the bull case becomes more likely.
