Pacific Health Care Organization provides workers' compensation medical-cost management services in California — health-care organization (HCO) and medical provider network (MPN) administration, utilization review, medical bill review, and medical case management for insurers, third-party administrators and self-insured employers. Utilization review is legally required for every workers' comp claim in California, and the company's subsidiary holds the URAC accreditation needed to perform that work — a real barrier that limits who can compete for this business. The company carries no debt. Its balance sheet holds about $12 million in cash and short-term investments against roughly $13.5 million in total assets and under $0.4 million in total liabilities, so a meaningful share of its reported earnings comes from interest on that cash position as well as from the underlying services business. Revenue is concentrated: in fiscal 2025, two customers together accounted for about a third of total revenue, and four customers made up two-thirds of accounts receivable. The company has lost large customers before, generally to competitors' lower pricing or bundled offerings, and says this risk will continue. That concentration, not debt or accounting complexity, is the main threat to the value of the shares. Owner earnings — operating cash flow after removing the swings from working-capital timing and after funding the modest capital spending the business needs to keep running — have ranged from about 3 cents to about 11 cents per share over the last ten years, moving with claims volumes tied to the broader economy. They fell sharply during the pandemic, when workplace injury claims dropped, and have since recovered to a new high in 2025, helped by a 42% jump in medical case management revenue. We used the ten-year median, about 6.5 cents per share, as the starting point for valuation — a figure that already absorbs the pandemic trough and does not assume the strong 2025 result repeats. Base case assumes revenue keeps growing at roughly the modest pace it has managed over the last decade after one above-trend year, then holds at that real level indefinitely, supported by the legal requirement for utilization review and the accreditation barrier that limits new entrants. That produces a value of about $1.30 per share. Bear case assumes pricing pressure continues and one of the two largest customers is lost within the next two years, similar to losses the company has experienced before, stepping earnings down by roughly a quarter with no recovery assumed afterward. That produces a value of about $0.76 per share — still solidly positive, because the UR/MPN/case-management business and the interest income on a substantial, debt-free cash position provide a floor even in a weak scenario. Bull case assumes the strong momentum in medical case management seen in 2025 continues for a few years before settling back toward the long-run trend. That produces a value of about $1.71 per share. The margin-of-safety price is around $0.80 — a discount large enough to absorb the customer-concentration risk described above, with the base case still implying meaningful upside from that level. No further business condition needs to be met first; the risk is already visible in the filings and is reflected in that price. What could make this wrong: if the two large customers stay indefinitely and the business keeps growing revenue the way it did in 2025, the shares are worth more than the base case suggests. If either customer leaves, or pricing competition worsens, the bear case is closer to right. We were not able to fully reconcile the jump in medical case management revenue to a specific new contract from the filings alone — it is described only as a general increase — so we treated that growth as not yet a repeatable, evidenced trend for the base case.
