MBX Biosciences is a clinical-stage biotechnology company developing long-acting injectable peptide drugs. Its lead program, canvuparatide, is a once-weekly treatment for chronic hypoparathyroidism, with a Phase 3 trial of roughly 160 patients planned to start in the third quarter of 2026. It also has an obesity candidate, MBX 4291, in Phase 1, with 12-week multiple-dose results expected in the fourth quarter of 2026. The company has no approved product and no product revenue, so it consumes cash: operating cash flow was about $(79.9) million in 2025 and $(54.7) million in 2024, and after stock compensation, working capital and equipment purchases our owner-earnings measure was about $(92.6) million in 2025. The 2025 net loss was $87.0 million. What the shares are worth today therefore rests on the cash the company holds, not on profits. At June 30, 2026 it held roughly $418.6 million of cash and marketable securities against total liabilities of $26.4 million, with about 48.2 million shares outstanding. Cash less liabilities is about $392 million, or roughly $8.14 per share, before any further spending. Each case below asks how much of that is left after the story it describes plays out. We give no value to the drug candidates themselves, because none has an approval, a partner contract or a completed pivotal trial. Canvuparatide would also enter a contested market: Ascendis Pharma's once-daily Yorvipath is already approved in the United States and European Union for hypoparathyroidism, the company's filing notes that Yorvipath's EU market exclusivity could run until 2035 at the latest, and another candidate, eneboparatide, has completed Phase 3. We did not use the company's own timelines to add value. Bear case, about $2.2 per share: the company spends roughly $110 million a year for two and a half years running the Phase 3 trial and its other programs, versus about $80 million of operating cash burn in 2025 and about $43 million in the first half of 2026. The trial does not produce a marketable drug, and about $10 million is spent winding down, leaving about $107 million. Base case, about $4.0 per share: spending averages about $100 million a year for two years while the Phase 3 trial reads out and other programs are trimmed. Nothing is credited for a positive result, so what remains is about $192 million. Bull case, about $4.6 per share: spending stays near the recent pace of roughly $85 million a year for two years and the trial runs on schedule, leaving about $222 million. A successful trial and approval could be worth far more than this, but that has not happened and is not credited. The main way this could be wrong is on the upside: an approved drug or a partnership would be worth much more than the cash. On the downside, the company may need to raise more money, which would dilute existing shareholders, and trials could cost more than assumed. Because most of the value is cash that is being spent, the margin-of-safety price is set below the bear case, that is, below the cash likely to remain even if the drugs fail. The 2024 per-share figure uses a partial-year share count around the company's initial public offering and is not comparable to 2025.
