Business condition attached to the margin of safety At least two consecutive quarters of renewed year-over-year total revenue growth, confirming the 6.6% revenue decline reported for the first half of 2026 was a temporary pause rather than the start of a longer decline.
Pulmonx makes the Zephyr Endobronchial Valve, a minimally invasive implant for severe emphysema, paired with two diagnostic products (the Chartis system and the LungTraX CT-analysis platform) that identify which patients will actually benefit before a physician implants anything. That diagnose-then-treat pairing is the company's real moat: its main rival, Olympus's Spiration valve, is sold without an equivalent patient-selection tool. Roughly 63% of 2025 revenue was booked in the United States, with the rest split across Europe/Middle East/Africa (about 30%) and Asia-Pacific and other international markets (about 7%); the international piece grew 23% in 2025 while the U.S. was nearly flat. The company has never turned a profit. Owner earnings - cash generated by the business after backing out working-capital swings, stock compensation, and the capital spending needed just to maintain the business - have run negative every year since at least 2018, and were roughly -$1.28 to -$1.52 per diluted share in each of the last five years, with the deepest losses in 2022-2023 and modest improvement since. That improvement stalled in the first half of 2026: total revenue fell about 7% year-over-year, reversing 2025's 8% growth (itself already a sharp deceleration from 22% growth in 2024). We could not find a specific, dated explanation for the 2026 slowdown in the filings available to us; it shows up in the reported numbers but nothing in the most recent annual report names a cause, so we've treated it as a real, recent fact without guessing at its source. One accounting judgment matters here: stock-based compensation (about $21 million in 2025) is treated as a genuine cost, not added back as "non-cash." A number of companies at this stage present an "adjusted" profitability figure that ignores it; we don't, because equity comp dilutes real shareholders exactly like a cash expense would. On that fully-loaded basis, the business is farther from sustainable profitability than adjusted figures would suggest. In March 2026 the company refinanced its bank debt with a new $40 million term loan (up to $60 million available) carrying a cash interest rate of roughly 11% - several points more expensive than the facility it replaced. Two additional $10 million tranches only unlock if trailing twelve-month revenue reaches $92.5 million and then $100 million by late 2027; on the last twelve months of reported results, revenue sits below the first hurdle and moving the wrong way. The company also disclosed that about 75% of its addressable patient population is Medicare-dependent, and it manufactures every product at a single facility in California - both are concentrations worth knowing about, though neither is a live crisis today. The company holds about $56 million of cash, has no formal going-concern qualification from its auditors, and management states it believes existing cash plus the new financing covers at least the next twelve months. Three cases: Bear case: The 2026 revenue softness is not a blip - volume continues to erode for a couple of years before leveling off around $80 million, well below 2025's $90.5 million. Management cuts commercial and R&D spending meaningfully to slow the bleeding, but even after those cuts the business runs a persistent loss because its cost base doesn't shrink as fast as revenue. Continued cash burn forces the company to keep issuing new shares to fund itself, diluting existing holders year after year. We don't extend this loss into perpetuity - beyond the ten-year horizon the outcome is genuinely unknowable in the bear case, so we assign no further value there. Result: roughly -$5.25 per share - the dilution and ongoing losses are severe enough that, on this path, the equity is worth less than nothing. Base case: The 2026 decline does not fully reverse - we hold year-one growth at or below the 2025 actual rate given two consecutive soft quarters, with no specific evidence to justify assuming an immediate rebound. From there, modest mid-single-digit growth resumes, carried mainly by the international expansion already underway, while U.S. sales stabilize. Gross margin holds near its historical 74-75%. Operating expenses grow more slowly than revenue, consistent with the discipline management already showed in 2025 when selling and administrative costs actually declined slightly even as revenue grew 8%. On this path the business does not reach sustainable profitability within the ten-year explicit forecast; it gets close by around year thirteen to fourteen and generates modest profits after that. Result: roughly -$4.80 per share - still negative, because the near-term losses (heavily weighted by discounting) outweigh a profitable but distant tail. Bull case: International growth continues near its demonstrated 2025 pace for several more years off a still-small base, the U.S. stabilizes rather than re-accelerating (we have no specific evidence for a U.S. rebound, so we don't assume one), and the additional $20 million of non-dilutive debt becomes available as revenue clears the contractual thresholds already in the credit agreement, avoiding further share issuance. Combined with continued cost discipline, the business crosses into profitability around year nine and reaches a healthy margin over the following years. Result: roughly +$1.65 per share. Even the optimistic case is worth only modestly more than zero today, because the discounted value of a decade of losses is large relative to a distant profit stream. The margin-of-safety price is a low single-digit figure (around $1.50 or less), and it applies only after at least two consecutive quarters of renewed year-over-year revenue growth confirm that the 2026 softness was temporary rather than the start of a longer decline. Absent that confirmation, the estimate does not support a margin-of-safety price.
