Lantheus makes diagnostic imaging agents. Its largest product is PYLARIFY, a PET imaging agent for prostate cancer, which brought in $989 million of $1,542 million in 2025 revenue (64%). The rest comes mainly from DEFINITY, an ultrasound contrast agent ($330 million), Neuraceq, an Alzheimer's-related brain imaging agent acquired with Life Molecular Imaging ($51 million), TechneLite generators ($87 million) and partnership, royalty and contract-manufacturing revenue, which includes Evergreen, acquired April 1, 2025. About 95% of revenue is booked in the United States. How we measure earnings. We start from operating cash flow, remove swings in working capital, and deduct stock compensation and the part of capital spending needed just to stand still. On that basis owner earnings per share were $0.92 in 2019 and $0.23 in 2020, then jumped after PYLARIFY reached scale: $4.64 in 2022, $5.60 in 2023, $5.91 in 2024 and $4.57 in 2025 ($313 million). Non-cash charges such as amortization and impairments are added back automatically; because the acquired assets do wear out, that wear is handled through the fading paths below rather than by changing today's earnings. Which level we use. The company is a different business from before 2022, so only 2022 onward counts. We do not use 2023 and 2024 as the normal level: PYLARIFY had a Medicare transitional pass-through payment from January 1, 2022 to December 31, 2024, and that boost has ended. Since January 1, 2025 payment is based on a lower cost-based rate, and the 10-K notes that a competing PSMA agent gained pass-through status on October 1, 2025 and is paid at average sales price plus 6%, which gives hospitals a financial reason to choose it. We anchor on the 2025 level of about $4.57 per share. First-half 2026 filings show owner earnings per share about 7% below the first half of 2025 ($2.14 versus $2.31), so we do not lift the level. Growth check. Owner earnings per share fell about 23% in 2025 (from $5.91 to $4.57). Our base case assumes a decline of about 6% in year one, less steep than 2025, because the one-time reset from the end of pass-through payment is already in the 2025 base and the first half of 2026 fell about 7%. No growth is credited anywhere: new products, Neuraceq expansion and any reimbursement improvement are not yet secured, so they are left out. Base case, about $39 per share. We split the $4.57 in proportion to revenue: about $2.93 tied to PYLARIFY and $1.64 to everything else. This split is our simplifying judgement; the company does not report profit by product. PYLARIFY earnings shrink about 10% a year indefinitely as pricing and competition erode them. PYLARIFY's regulatory exclusivity expired May 26, 2026, but the company lists six patents, the last expiring in 2037, and reported no generic challenge notice as of its February 2026 10-K, so we do not assume an abrupt collapse. The other products stay flat for 30 years and are then worth nothing: DEFINITY revenue rose from $280 million in 2023 to $330 million in 2025, while TechneLite fell 9% in 2025, so we credit neither growth nor decline. Discounted at 6%, PYLARIFY is worth about $16.5 and the rest about $22.6 per share. Bear case, about $18 per share. A generic or competing version challenges the patents, and PYLARIFY earnings fall 25% a year (about $7 per share). The rest also erodes, 5% a year (about $14), reflecting TechneLite's 2025 decline. The company owes $575 million of 2.625% convertible notes due December 2027; we assume refinancing at materially higher rates costs about $3 per share. The patent lawsuit with Novartis over the company's generic lutetium product, on which a decision from the December 2025 trial was still pending at filing, is not counted in any case. Bull case, about $51 per share. The 2037 patents hold and PYLARIFY earnings fall only 4% a year (about $28), while the rest stays flat for 30 years (about $23). We do not assume payment-rate relief or new products. What we tested. The main assumption is a 10% annual PYLARIFY decline. It looks severe against the 6.5% PYLARIFY revenue decline in 2025, but total owner earnings fell 23% that year and the first half of 2026 fell further, so we kept it. Leaning the other way, the filing reports no generic challenge notice yet, which is why the base case is not steeper. What would make this wrong. A faster generic entry or wider payment gap versus competitors would push value toward the bear case; success in getting cost-based payment replaced by average-sales-price payment would push toward the bull case. Cash and debt are not added to or subtracted from these values, because owner earnings already reflect interest paid. We could not review the second-quarter 2026 filing text directly, only its reported financial data.
