bear $67.08 base $147.20 bull $226.65 margin of safety $79.00 market price* $126.70
Business condition attached to the margin of safety No disclosed program loss, in-sourcing decision, or material volume reduction from any of Integer's three largest customers (together ~49% of 2025 sales, with the largest alone at 20% and rising).
Integer Holdings is the leading outsourced manufacturer of components and finished devices for cardiac, vascular and neuromodulation implants, plus a smaller book of other medical and non-medical work. It builds parts - batteries, leads, catheters, electronic subassemblies - that go inside pacemakers, neurostimulators, structural-heart devices and vascular tools sold under the brands of large device makers. Once a device using an Integer part is FDA-approved, switching suppliers means re-validating the whole device, which is what gives these customer relationships their durability. The business runs as three product lines. Cardio & Vascular is now about 60% of sales and the growth engine, up 17% in 2025 on new electrophysiology and neurovascular product ramps plus a handful of bolt-on acquisitions (even excluding those acquisitions, the segment's organic growth was still north of 10%). Cardiac Rhythm Management & Neuromodulation is about 36% of sales and growing only modestly, because a legacy spinal-cord-stimulation program the company flagged as winding down back in 2020 has now largely rolled off. Other Markets is the smallest piece at about 4% of sales and shrinking quickly - Integer announced in 2022 that it was deliberately exiting its Portable Medical battery business over several years, and that planned exit is most of why this segment fell 27% in 2025 and should keep shrinking toward zero over the next couple of years. Owner earnings - cash generated from the business after stripping out working-capital swings, stock-based compensation, and the portion of capital spending used to grow rather than simply maintain operations - have run from about $4.55 per diluted share in 2019 to $3.30 in the pandemic-affected 2020, back up into the $3-4 range through 2021-2024 (a period that also included a step-up in capital spending to build new Cardio & Vascular manufacturing capacity), and then to $5.89 in 2025 (about $4.58 excluding a one-time non-cash charge tied to this year's convertible-debt refinancing). We used the median of the last seven years, $3.88 per share, as the normalized starting point rather than the strong 2025 figure, since 2025 benefited from an unusual mix of acquired revenue and a large non-cash refinancing item unlikely to repeat. Three cases, each a continuous story rather than a percentage of another: Bear case, about $67 per share: one of Integer's three largest customers - together nearly half of 2025 sales - meaningfully cuts or in-sources a program, something that has already happened twice in disclosed form (the 2020 neuromodulation wind-down and the 2022 Portable Medical exit). Growth turns modestly negative for a couple of years before settling near 1% annually, while roughly $1.2 billion of debt (convertible notes plus a credit facility, costing about $27 million of interest in 2026) continues to be serviced from a smaller cash-flow base. Base case, about $147 per share: Cardio & Vascular keeps compounding at a high-single to low-double-digit rate for several years, Cardiac Rhythm Management & Neuromodulation grows at a market rate of low-single digits, and the Portable Medical exit finishes running off by around 2027. Growth decelerates gradually toward roughly 4% annually by the back half of a ten-year forecast, and we assume the business's long, demonstrated record of quality-controlled manufacturing for regulated medical devices supports modest continued growth (2.5% a year) after that, reflecting the switching-cost relationship with customers rather than any specific new contract. Bull case, about $227 per share: the current wave of new-product ramps in electrophysiology and neurovascular devices continues for longer and at a faster pace, the legacy CRM&N headwind is fully behind the company, and bolt-on acquisitions keep adding to Cardio & Vascular at roughly the pace seen in 2025 - all things Integer has already shown it can do, not a hoped-for change in the story. What could make this wrong: customer concentration is real and increasing - the single largest customer grew from 16% to 20% of sales over the last three years - so one customer's decision could move results more than at a typical peer. The company also took on real leverage this year through two rounds of convertible-note financing, adding fixed interest costs ahead of any return to shareholders. On the other side, if Cardio & Vascular's current growth simply continues, the base case could prove conservative. The margin-of-safety price near $79 reflects a wide margin of safety against the bear case, given how concentrated the growth is in a handful of customers and product launches and given the debt load. It also assumes no disclosed loss of business from any of the three largest customers.

Sales geography
Products ship mainly to customer (OEM) receiving facilities in the United States (about 53% of 2025 sales), with the remainder to Costa Rica (~9%) and Puerto Rico (~7%, both established medical-device manufacturing/distribution hubs for large device makers), Ireland (~5%), and the rest of the world (~25%), reflecting where large medical-device customers take delivery rather than end-patient geography. Integer's own manufacturing footprint is global: 21 facilities in the United States, 3 in Mexico, 3 in Ireland, and one each in the Dominican Republic, Uruguay, Malaysia, Costa Rica, Switzerland, and China (the China site is a small part of a 30-facility network).
