Business condition attached to the margin of safety At least two consecutive quarters of demonstrably positive owner earnings from the core Theranostics/Cobalt/Calibration segments (or a successfully commercialized Medical Device segment) without relying on working-capital timing benefits, and/or resolution (sale or meaningful cost reduction) of the dormant uranium de-conversion facility license.
Radnostix Inc. (formerly International Isotopes Inc., trading as INIS on the OTCQB) operates in nuclear medicine: it manufactures cobalt-60 sealed sources and calibration/reference standards used in radiation-therapy equipment, plus sodium iodide I-131 products for cancer treatment (its Theranostics segment). It is also building out a Medical Device segment (EasyFill, RadVent, and products acquired from AMICI) and holds a long-dormant Nuclear Regulatory Commission license for a planned uranium de-conversion facility in New Mexico that it has been unable to monetize -- a deal to sell those assets for roughly $12.6 million was terminated in March 2026 after the buyer could not complete payment. Looking at cash the business actually generates after stripping out non-cash accounting items, stock-based pay, and the capital spending needed just to keep existing operations running (as opposed to growing them), this company has hovered between modest profit and modest loss for the past eight years, with results driven mostly by the timing of receivables, payables and inventory rather than any clear trend of improvement. Revenue has grown gradually from about $10 million to a peak of roughly $14 million, but that growth has not translated into durable profitability once real reinvestment is counted. Our base case assumes this pattern continues: revenue in the $12-14 million range, thin-to-negative underlying cash generation, offset by continued share issuance to fund shortfalls (share count has grown from about 411 million to about 527 million over the past seven years). We did not assume management's stated plans -- new sales agreements, cost controls, new products -- lift results meaningfully above the historical pattern, because none of those plans carries a specific date or number yet. Our bear case assumes the newer Medical Device products fail to gain real traction, competition in cobalt and calibration products continues to pressure margins, and the company keeps carrying the cost of the still-unsold uranium facility license with no buyer, funded by increasing share dilution. Our bull case assumes the Medical Device segment succeeds commercially and the company either sells the uranium facility assets or meaningfully cuts the cost of carrying them, allowing the business to reach modest, sustained profitability. In this scenario, most of the estimated value comes from what the business could earn in the years beyond our ten-year forecast, so it is more sensitive to whether the newer product lines pan out than to near-term results. Because there are more than 500 million shares outstanding, all of these values are small on a per-share basis. Our base case is modestly negative (roughly -$0.014/share): historically, the cash this business needs has slightly exceeded what its operations generate once real reinvestment is included. This is not a case of debt exceeding what the company is worth -- the balance sheet carries more cash than debt -- it simply means the operating business has not been reliably creating value for owners by this measure. Our bear case is roughly -$0.036/share and our bull case roughly +$0.016/share. Given the base and bear cases are at or below zero, no positive margin-of-safety price applies without first a real change in the underlying business: specifically, at least two consecutive quarters in which the core segments generate positive cash after real capital spending, without help from working-capital timing. One item we flag rather than resolve: the most recent annual filing includes a note on business condition and liquidity discussing a history of losses and a $128 million accumulated deficit. The independent auditor's opinion on those statements did not include a going-concern qualification, but we treated the underlying risk as real and built it into our bear and base cases rather than assuming it away.
