Guardian Pharmacy Services dispenses medication to residents of assisted living, behavioral health, and skilled nursing facilities, serving roughly 205,000 residents across about 8,400 facilities in 38 states through 61 pharmacies. Founded in 2004 and taken public in September 2024, the business has grown for two decades through a combination of adding residents at existing pharmacies and buying small, independent long-term-care pharmacies, funded from its own cash flow rather than debt - the balance sheet currently carries no meaningful borrowed money. Reported net income has been an unreliable guide to the company's earning power in its first two years as a public company. A one-time, non-cash charge tied to the 2024 stock reorganization around the IPO produced a large accounting loss that year, even though cash generated by the business was solid ($58 million). By 2025, that legacy charge had mostly faded and cash generation improved to $100 million on revenue of $1.45 billion, up 18% (12% from more prescriptions and drug-price inflation at existing pharmacies, and 6% from newly acquired ones). More recently, growth has slowed noticeably - revenue in the first half of 2026 was up only about 2% from a year earlier, well below the 18% pace set in 2025. We could not find a specific, dated explanation for this in the filings available to us. Encouragingly, profit margins actually improved over the same stretch, which suggests the slowdown looks more like a pause in the pace of new pharmacy acquisitions - which happen in lumps, not on a schedule - than a sign the underlying business is deteriorating. We have not assumed the faster historical growth returns without more evidence, and we flag this pause plainly. One dated risk worth noting: starting in 2026, new federal rules (the Medicare Drug Price Negotiation Program under the Inflation Reduction Act) set government-negotiated prices for a first group of high-cost drugs, with more drugs added in later rounds. This could pressure the economics of dispensing certain medications over time, though it has not shown up in the margin so far. We valued the business under three cases, each starting from 2025's underlying cash-earning power. Growth for 2026-2027 is assumed close to the recent, slower pace, since we have no confirmed reason to expect an immediate rebound; growth beyond that continues modestly, tied mainly to prescription volumes and drug-price increases that have historically kept pace with the business. - A cautious case assumes the recent slowdown persists and reimbursement pressure bites, with owner earnings settling roughly 5% below the 2025 level and staying flat from there. That case is worth about $17 per share, once cash on the balance sheet is added in. - Our central case assumes earnings grow modestly (around 4% a year) as the business finds a steady rhythm between organic growth and a slower but ongoing pace of small pharmacy acquisitions. That case is worth about $31 per share. - An optimistic case assumes the company returns closer to its demonstrated 2025 growth rate, tapering over time as the pace of acquisitions naturally slows relative to the growing size of the business. That case is worth about $39 per share. The margin-of-safety price is around $15, which sits below even the cautious case, to leave room for the risks noted above - especially the company's short public trading history and the 2026 drug-pricing changes whose effect is not yet visible in the numbers. A modeling note: the large 2024 stock-compensation charge tied to the IPO reorganization was treated as a one-time item and excluded when setting the starting earnings level, since it will not recur; residual amounts are still working their way out of the 2025 figure and should shrink further from here.
