Business condition attached to the margin of safety Two or three more quarters showing Epioxa (FDA-approved October 2025, commercial launch begun Q1 2026) generating meaningful, growing sales alongside continued improvement in operating cash flow toward breakeven, with the positive swing already visible in the first half of 2026 continuing rather than reversing.
Glaukos is an ophthalmic medical device and pharmaceutical company built around two franchises: glaucoma (roughly 83% of 2025 sales), led by its iStent family of micro-invasive glaucoma surgery implants and the newer iDose TR sustained-release implant, and corneal health (roughly 17% of sales), centered on corneal cross-linking therapy for keratoconus. About three-quarters of sales are generated in the United States, with the remainder international. The company has invested heavily to build this position and has not yet turned that investment into cash profit: on the measure used here (operating cash flow after removing the swings caused by working-capital timing, stock-based compensation, and the portion of capital spending that funds growth rather than upkeep), owner earnings per share have been negative in each of the last six years, ranging from roughly break-even in the best of those years to a loss of over $2 per share in the worst (2023). Two developments explain both the recent loss years and the recent improvement. First, the company completed a multi-year investment cycle: a large one-time facility purchase in 2021, and heavy R&D and launch spending behind iDose TR (approved December 2023, launched February 2024) and Epioxa (approved October 2025, launch begun in the first quarter of 2026), a next-generation, incision-free cross-linking therapy that succeeds the older Photrexa product. Second, because Epioxa is replacing Photrexa, the company concluded in 2025 that Photrexa's remaining commercial life was under a year and wrote off substantially all of the $112.9 million book value of that intangible asset as a one-time, non-cash charge. That charge depressed reported 2025 profitability but does not represent an ongoing cash cost, and it has already been correctly excluded from the owner-earnings figures used here. The more important fact is that cash generation has been improving in a way that is corroborated by more than one data point: operating cash flow moved from a $61 million use of cash in 2024 to a $15 million use in 2025, and turned modestly positive in the first half of 2026 -- the first time that has happened in several years. Stock-based compensation, while still a real cost that is deducted here in full, has also been shrinking steadily as a share of revenue (from about 21% in 2020 to about 12% in 2025) as the business scales. These are the specific, dated facts the three cases below are built from. The base case assumes this improvement continues, tracking roughly the pace already achieved from 2024 to 2025: owner earnings move through breakeven within the next year or two as Epioxa and iDose TR both scale, then build toward a mature, profitable run rate over five to seven years as gross margin normalizes (2025's unusually low margin was a direct result of the one-time write-off) and the historical decline in compensation costs as a share of revenue continues. Beyond that, the base case leans on the company's demonstrated, decade-long record of successfully launching successive product generations (four iStent generations plus iDose TR, each sustaining or growing the glaucoma franchise) as the basis for modest continued growth, and on the fact that only about a quarter of sales are currently generated outside the U.S. despite the company's products recently receiving European regulatory certification, leaving room to grow that share. The bear case assumes Epioxa's launch proves harder than iDose TR's did -- it has only a few months of real commercial history as of this analysis, unlike iDose TR, which had two full years of data behind its 2025 contribution -- and that working capital keeps consuming cash at anything like recent rates as new-product inventory builds. In this case profitability arrives only slowly, over five to six years, and the business is worth meaningfully less than the base case, though it does not face any near-term liquidity or solvency problem. The bull case assumes both new products execute the way the company's device franchise has historically executed, reaching sustained profitability within a year or two and growing at a healthy clip thereafter as international sales catch up to the U.S. business's scale. Given how much of the base and bull cases still depends on a launch (Epioxa) that only began generating sales this year, the margin-of-safety price carries a substantial discount to the base-case value, and it depends on a few more quarters confirming the cash-flow turn before the improving trend is treated as established.
