Denali Therapeutics develops medicines designed to cross the blood-brain barrier. Its first product, AVLAYAH (tividenofusp alfa-eknm), received U.S. accelerated approval on March 25, 2026 for the neurologic manifestations of Hunter syndrome (MPS II) in children. Approval rests on a reduction in a spinal-fluid biomarker, and continued approval depends on the COMPASS confirmatory trial showing clinical benefit. The label carries serious warnings, including hypersensitivity and anaphylaxis, anemia and membranous nephropathy. The business does not yet earn money. Our measure of owner earnings (operating cash flow, less working-capital swings, less stock compensation, less maintenance capital spending) was about negative $522 million in 2025 (negative $3.02 per share), negative $483 million in 2024 (negative $2.94 per share) and negative $176 million in 2023 (negative $1.28 per share). The 2023 figure is helped by the Biogen collaboration: $295.5 million of that year's $330.5 million of revenue came from Biogen, and most of it was the recognition of a payment received earlier, which we removed as a working-capital effect. Stock compensation of roughly $100 million a year is treated as a real cost. In the first half of 2026, cash used in operations was $228.2 million, versus $206.8 million a year earlier, so losses are not yet shrinking. AVLAYAH's first full quarter of sales (the second quarter of 2026) was $3.6 million. Management expects $10 to $12 million for the third quarter, but that is a forecast and we do not count it. With no positive earnings to anchor on, the value of a share comes mainly from the cash the company holds. At June 30, 2026 Denali reported about $940 million of cash and marketable securities. In July 2026 it sold the priority review voucher it received with the approval, and it reported liquidity of more than $1.1 billion after that sale. We use $1.1 billion, or about $5.87 per share on roughly 187.3 million shares, and treat the sale as a completed fact. Spending at twice the first-half 2026 pace is about $456 million a year, so this cash covers roughly two and a half years. One obligation matters. In March 2026 Denali received $200 million from Royalty Pharma in return for 9.25% of future net sales of AVLAYAH, with payments ending once a total of 3.0 times the funding has been paid (2.5 times if reached by the first quarter of 2039). We credit no meaningful sales in our cases, so no payments are deducted, but any real success for the drug will be shared with Royalty Pharma. Bear case, about $1.00 per share: spending continues at the current pace for two more years with sales far below the cost of running the company, leaving about $188 million, or $1.00 per share, from the $1.1 billion. The accelerated approval could be withdrawn if the confirmatory trial fails, and nothing else is credited. Base case, about $3.44 per share: spending continues at the first-half 2026 pace for one more year, using about $456 million and leaving about $644 million, or $3.44 per share. We do not count AVLAYAH sales as an offset because one quarter of $3.6 million is not a record we can build on. We do not credit the wider pipeline, since none of it is secured or repeatable today. Bull case, about $5.87 per share: launch sales and cost control are good enough that the company keeps the $1.1 billion it holds today. The launch would have to fund the company's own spending, which is far beyond what has been shown so far. What would make this wrong: if AVLAYAH sales grow well beyond one quarter's evidence, or the pipeline delivers approved products, the shares would be worth more than our figures. If spending rises or sales stall, less. Because the value rests mostly on cash that is being spent, the margin-of-safety price sits well below these values.
