Business condition attached to the margin of safety Quarterly PALSONIFY sales covering most of the company's cash operating spending, so that the yearly cash burn falls well below the roughly $440 million annual pace seen in the first half of 2026, for at least two consecutive quarters.
Crinetics Pharmaceuticals is a drug developer that has just become a drug seller. The FDA approved its first product, PALSONIFY (paltusotine), on September 25, 2025, as the first once-daily oral treatment for adults with acromegaly who did not respond adequately to surgery or cannot have it. Sales began in October 2025. Behind it are atumelnant (in Phase 3 for congenital adrenal hyperplasia), paltusotine in Cushing's syndrome (Phase 3), and earlier programs. The company has reported a net loss in every year we reviewed, 2017 through 2025. What the business earns today. We measure the cash a business generates for its owners after the cost of stock compensation and the equipment needed to stay in business. On that basis Crinetics has lost more each year: about $2.42 per share in 2020, $2.82 in 2021, $3.01 in 2022, $3.80 in 2023, $3.85 in 2024 and $5.13 in 2025. Cash used in operations was $378 million in 2025 and $219 million in the first half of 2026 alone, roughly $440 million a year. Stock compensation of $91 million in 2025 is treated as a real cost. Losses grew about 33 percent per share from 2024 to 2025, and we assume no improvement in the first year, because no earnings history exists to support one. Sales are rising quickly: $5.4 million of PALSONIFY revenue in the fourth quarter of 2025, and $25.1 million of revenue in the second quarter of 2026 (about $35.9 million for the first half). That is a strong start, but it is two quarters of a new launch, and it is still far below the roughly $440 million a year being spent. What the shares hold that we can count. The company reported about $1.0 billion of cash and investments at the end of 2025 and roughly $1.4 billion after raising about $380 million from a January 2026 share sale. Subtracting the first-half 2026 cash burn leaves an estimated $1.18 billion, about $11 per share on the 106.1 million shares outstanding in July 2026. This is an approximation, not a reported figure. The cash is real, but it is being used to fund losses, so it does not belong to shareholders unless the spending slows. We found no borrowings in the filings that change this picture. Bear case, $0 per share. Spending stays near the current pace and PALSONIFY sales stay near the second-quarter level of about $100 million a year. The cash lasts about three years, and any new funding would come from issuing more shares. Nothing that we can point to today shows the product alone covering costs of this size. Base case, $3.50 per share. The same spending pace continues for two years, which uses up about $0.8 billion of present value, leaving about $0.37 billion, or roughly $3.50 a share. After that we credit nothing: we can name no contract or track record showing when losses end, so the value beyond cash still on hand is zero. We did not credit continued growth in PALSONIFY sales, European approval (a positive regulatory opinion was issued in February 2026, but a final decision and sales are not yet in hand), or the Cushing's and adrenal programs, because none has yet produced results as a sold product. The base case is therefore the cash left after a defined period of losses, not a forecast of profits. Bull case, $9.60 per share. Losses end within about one year, which the second-quarter jump in sales, more than 125 prescribers and more than 200 enrollment forms received in the first months after launch support as a possibility. That leaves about $7.20 of cash per share. We add about $2.40 for the company's stake in Radionetics, a company to which it licensed its radiotherapy technology and in which it held roughly 25 percent at the end of 2025. In July 2024 Radionetics gave Eli Lilly the exclusive right to acquire it for $1.0 billion; roughly a quarter of that price is about $250 million. Lilly has no obligation to complete the acquisition, so this is included only in the bull case. The patents. The company's patents on the paltusotine compound itself are estimated to expire in 2037 before any extension, and further patents on its use, dosing and forms are estimated to run between 2039 and 2046. U.S. extensions are capped at 14 years after approval. Protection is therefore limited in time, which argues for caution on any value that depends on long-term sales. What would make this wrong. The valuation could be too low if PALSONIFY sales keep roughly doubling each quarter and the company chooses to slow its spending. It could be too high if sales flatten or cash is used faster than in the first half of 2026. We could not tell from the filings how much of the current spending is optional, and we did not count it as such. Because the cases depend on an unusually wide range of outcomes and no earnings history, the margin-of-safety price is set with a large margin, around $1.00 per share, together with a condition that sales are clearly closing the gap with spending.
