BioAge Labs develops drugs aimed at the biology of aging and chronic metabolic disease. It has no product on the market and has never had profits. Its lead candidate is BGE-102, an oral NLRP3 inhibitor for inflammation-driven cardiovascular risk and diabetic macular edema, moving from early into mid-stage trials in 2026. Its only revenue is $9.0 million in 2025 of collaboration revenue under a December 2024 agreement with Novartis, and it also works with Eli Lilly on earlier-stage programs. What the business earns today: nothing. Cash from operations was negative $81.6 million in 2025 and negative $51.5 million in 2024. After removing timing swings in receivables, payables and deferred revenue, and after charging stock-based pay ($11.7 million in 2025) and about $0.7 million of equipment purchases, owner earnings were about negative $82.2 million in 2025 (negative $2.29 per share) and negative $70.0 million in 2024 (negative $6.53 per share, on a share count that was small until the September 2024 IPO). The loss is widening: owner earnings worsened about 17% from 2024 to 2025, and operating cash outflow in the first half of 2026 was $43.3 million against $37.3 million a year earlier, so we assume spending stays flat at about $87 million a year rather than easing. Because there is no earnings stream, we value the shares on the cash they hold and how long it is spent before anything else is proven. The cash: at June 30, 2026 the company held about $319 million of cash and marketable securities (about $74.0 million cash and $244.8 million marketable securities), after raising $107.6 million in a January 2026 stock offering at $19.50 a share. With 45.9 million shares outstanding, that is roughly $6.94 per share before any spending. The annual report states that cash on hand is sufficient to fund planned operations for at least a year, so we do not see near-term funding stress. What we left out: the value of the drugs succeeding. BGE-102 showed an 86% median reduction in an inflammation marker (hsCRP) in obese participants in early trials, but the 160-patient Phase 2a trial has not read out (results expected by the end of 2026) and the diabetic macular edema trial reports in mid-2027. The Novartis agreement carries up to $530 million of future milestones, but these depend on discoveries not yet made. The company's first lead candidate, azelaprag, was discontinued in December 2024 after liver enzyme elevations in a Phase 2 trial, which is a reminder that these outcomes are uncertain. Since none of this is secured or repeatable, we assign it no value. The result understates the payoff if the drugs work. Bear case, $1.08 per share: the company keeps spending at about $87 million a year for three years without a program that changes its prospects, and about $59 million (worth roughly $49 million today) remains. Azelaprag is the evidence that spending can be lost to a failed program. Base case, $2.82 per share: spending continues for two years, through the two trial readouts and a further year of follow-on work, leaving about $146 million (about $130 million today). Bull case, $4.77 per share: only the trials already planned are funded, about one year of spending, leaving about $232 million (about $219 million today). This is not a success case; it is the outcome in which management stops spending when the current trials finish. All three cases are on today's 45.9 million shares. The company recorded $11.7 million of stock-based pay in 2025 and has sold shares repeatedly, so further dilution would lower these figures. What could make this wrong: positive Phase 2 results or a partner paying an upfront fee would be worth far more than these cases assume; on the other side, spending above the current rate would erode the cash faster. We could not review the narrative of the June 2026 quarterly report, so the cash and share figures come from its reported financial statements alone.
