BridgeBio is a commercial-stage drug company. Its one large product is Attruby (acoramidis), an oral pill for transthyretin amyloid cardiomyopathy (ATTR-CM), a heart condition. Attruby was approved by the FDA in November 2024 and is sold outside the United States as Beyonttra, through Bayer in Europe and Alexion in Japan. Attruby brought in $362.4 million of U.S. sales in 2025, and total revenue was $502 million. Revenue in the first half of 2026 was $438 million, with $244 million in the second quarter alone, against $111 million a year earlier. The company is still spending heavily on research and on launching more products, so it has burned cash in every year we can see: operating cash flow was negative $446 million in 2025, and after stock compensation was subtracted the shortfall was about $2.76 per share. HOW WE VALUED IT. We valued only what is already in hand: the Attruby franchise as it exists today. We did not count the three late-stage programs (infigratinib for achondroplasia, BBP-418 for limb-girdle muscular dystrophy, encaleret for ADH1). All three reported positive Phase 3 results and filings are planned, but none is approved, so we give them no value and we also leave out the bulk of the research spending that funds them. That makes these cases conservative on the pipeline; if the products are approved and sell, the shares would be worth more than we show. Stock-based compensation is treated as a real cost. Per-share figures use about 196 million shares. Convertible notes are not deducted as debt, but they matter for the downside (see the low case). BASE CASE: about $5.90 per share. Revenue holds at roughly $900 million a year, which is where the first half of 2026 was running. We do not assume further growth: the launch is only about 20 months old and has not been tested by a downturn. After the cost of goods (about 6% of sales), a 5% royalty on global acoramidis sales owed to the investors who funded the launch in 2024 (capped at $950 million in total), a share of the European royalties from Bayer that was sold to investors in 2025 (60% of Bayer's royalties on the first $500 million of annual European sales, capped at 145% of the $300 million paid), a selling and marketing cost of roughly $320 million, about $100 million of sustaining research, and about $80 million of stock compensation, we estimate roughly $200 million a year of owner earnings after light cash taxes. The company has about $1.7 billion of federal tax losses, usable against up to 80% of taxable income, so cash taxes stay low. We run this to 2033, the latest expiry among the composition-of-matter patents licensed from Stanford (some expire in 2031 and others in 2033, before any extension). Nothing after 2033 is credited: the salt-form and formulation patents run to 2038 and 2039 but we cannot show they would block a generic. Discounted at 6%, that is about $1.15 billion. LOW CASE: about $1.50 per share. Named risks: (1) sales stall at roughly $750 million because Attruby competes with Pfizer's Vyndaqel/Vyndamax (tafamidis) and, since 2025, Alnylam's vutrisiran; (2) the royalty on Attruby sales, which the funding agreement allows to rise from 5% to as much as 10% starting in 2027 if certain conditions are met, is charged at 10%; (3) protection ends in 2031, the earliest patent expiry, because the patent-term extension applications have not yet been granted. Selling costs are cut to about $280 million. That leaves about $80 million a year for five years. The company also carries convertible notes maturing between 2027 and 2033, including $550 million of 2027 Notes, $747.5 million of 2029 Notes and $632.5 million of 2033 Notes as originally issued. Under our method we do not deduct this debt, but a case this thin leaves little cushion for repaying or refinancing it, which is why this case is low. HIGH CASE: about $24 per share. Attruby's own momentum continues: quarterly revenue has more than doubled in a year, prescriptions came from more than 1,800 prescribers by February 2026, and in Germany Beyonttra reached more than 50% of new prescriptions in its first year. We assume revenue reaches about $1.5 billion by 2028 and stays there, and that the patent-term extension applications filed after approval are granted to the statutory limit of 14 years after approval, which would protect Attruby into late 2038. That produces roughly $600 million a year of owner earnings once the launch matures, with the two royalty burdens limited by their caps. We do not include a market recovery or any pipeline product. WHAT WOULD MAKE THIS WRONG. On the low side: physicians favoring the competing therapies, the funding-agreement royalty stepping up, or a generic entrant in 2031. On the high side: the first-half 2026 revenue rate is only two quarters of evidence, and selling costs of about $320 million are our estimate for running Attruby alone (selling and administrative spending was $289 million in 2024, the launch year, and $350 million in the first half of 2026 as the company prepares three more launches). If running the franchise needs much more than we assume, the base case shrinks quickly because the margin is thin. WHAT WE COULD NOT FULLY RECONCILE. The company's cash royalty repayments to investors are reported outside operating cash flow ($15.5 million in 2025), so they are not in our historical figures, but they are deducted in all three cases going forward. We do not know how much of the ex-U.S. revenue is Bayer royalty versus milestone payments, so the European royalty share is an estimate. Because the valuation is driven by a small number of assumptions with a wide range of outcomes, the margin-of-safety price sits at a large discount to the base case.
